Clinical Nurse Specialist for Acute Oncology and Lung Cancer
This is a new an exciting role, working alongside both the Acute Oncology Service (AOS) and the lung cancer team. We are looking for a passionate individual, willing to develop their skills in acute oncology and lung cancer. The AOS provides urgent assessment, rapid initial management and appropriate specialist input (ensuring access to appropriate disease site specific advice) for any patient presenting acutely with problems relating to cancer or its treatment. The essence of an AOS is to accelerate, coordinate and signpost patients on to the correct assessment and treatment pathway and ensure consistent access to oncology expertise, wherever the patient presents.
The respiratory service provide care for patients with lung cancer throughout their cancer journey. Nearly half of the acute oncology patients have a lung cancer diagnosis.
The post holder will work within the multi- professional team, as directed by the relevant Clinical Nurse specialist (Band 7), delivering care to Acute oncology patients and lung Cancer patients within the Cwm Taf Morgannwg University Health Board (CTMUHB). They will work closely with acute oncology team, the lung cancer team, families and carers to provide a co-ordinated and patient centred cancer nursing service.
Main duties of the job
The aim of the service is to provide rapid identification and appropriate prompt management of patients that present with:
- previously undiagnosed cancer.
- complications as a direct consequence of their cancer e.g. metastatic spinal cord compression, hypercalcaemia.
- acute complications following cancer therapy e.g. neutropenic sepsis.
The CNS will provide clinical support and expertise in acute oncology assessment in the emergency admission areas, provide early input of Specialist Oncologist assessment and clinical management advice for unscheduled oncology emergency admissions with regard to the following:
- Complications related to chemotherapy.
- Complications of cancer.
- Suspected unknown primary cancer.
- To take the lead with regard to MSCC, Neutropenic Sepsis and Cancer of Unknown Primary.
The post holder will share the responsibility for a defined caseload of Acute Oncology patients, with a focus on lung cancer and will be expected to triage the referrals on a daily basis in order to prioritise their workload. This involves independently reviewing referrals (new and existing) and assessing patients requiring acute oncology intervention, using information gathered from patient systems such as CANISC/WCP/Myrddin.
This post is fixed term/secondment for 2 years from start date, due to funding. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

Clinical Duties
Assess patients admitted as an emergency and ensure they are receiving appropriate care and investigations as per agreed pathways whilst actively integrating theory into practice.
Exercise professional autonomy and make critical judgements using knowledge and skills in order to provide the highest standards of holistic patient care.
Ensure the patient and, if appropriate their relatives, are involved with decision making and aware of proceedings.
Deliver evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care; act as an advocate for the patient.
Advise clinical teams of the appropriate investigations, diagnostic tests and imaging according to patient presentation, care pathways and clinical competence.
Respond to situations involving oncological emergencies using clinical judgement and decision making to initiate appropriate management.
Service Management
• As part of the Cancer multi-disciplinary team, assist in the development of care pathways for the Acute Oncology Service.
• Promote awareness across all Teams of the requirements for an Acute Oncology Service and its application, promoting and gaining support and providing expert knowledge to support its implementation and ongoing performance.
Service Improvement
• To develop services in order to meet local/national objectives and standards.
• To assist in continuing development and audit of the role to develop relevant changes in patient or departmental needs, ensuring a quality service is provided and maintained and implementing of service improvements from the results.
Communication
• Establish and maintain effective communication with patients, family and carers to assess needs, providing relevant support, information, education, advice and counselling as required for cancer related issues.
• Communicate effectively and sensitively with patients, including families/relatives/carers, who have a confirmed or suspected cancer diagnosis, often at times of distress, offering support throughout episodes of emergency admissions, including assisting through the initial cancer diagnosis and episodes of disease progression.
• Demonstrate skill in conflict resolution and competent negotiation skills when dealing with difficult or challenging situations.
• Provide co-ordinated specialist service through effective liaison with all members of the multi-disciplinary team within Primary, Secondary and Tertiary care settings as well as third sector organisations.
Personal and People Development and People Management
• Deliver education and training to various health care professionals and students regarding Acute Oncology and Oncological Emergencies.

This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
