Integrated Cluster Service Networks Teams
This post is a permanent position within the Integrated Network Services in the East of Bridgend. As a Team Leader within our Integrated services you are in the unique position to work within and lead a team who are fully focused on holistic person centred care, with a what matters to the individual and family approach. As a Specialist Practice and Public Health practitioner you will have a full multi disciplinary team of professionals at your fingertips to provide a full preventative health agenda whilst focusing on the continuous improvement of self, your team and community cluster your serve.
Main duties of the job
This position provides an exciting opportunity for highly motivated and ambitious District Nurse who is committed to excellence in care and the passionate leadership of District Nursing services.
The successful candidate will provide vision and energy in developing holistic integrated care and support for the patient and those who are important to them. The role requires effective management and strong leadership qualities. The post holder, will be skilled and creative, with high levels of autonomy and advanced clinical decision making. The post holders will need to work closely with GPs and surgery staff, and the wider multi-disciplinary staff to support individuals to have good quality emotional and physical well-being in their own homes.
The successful candidate will offer clinical leadership within the Network Team and provide support and professional development for their staff. They will oversee the operational management of the a given caseload, ensuring high quality care for patients and support for those important to them. As a team leader for the service you will be responsible for ensuring prudency, consistency and a positive work culture, where all team members feel valued and included.
