Job overview

We are redesigning our community services model and implementing a new Intermediate Care Team and therefore an exciting opportunity has arisen to employ Nurses to deliver an urgent community response services (provides care 8am to 9pm, 7 days a week) and will play a crucial role in providing essential support and care to individuals, with a focus on promoting independence, dignity, and well-being. This is an opportunity for someone who wishes to continue to progress their career in nursing.

The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or prevent admission with interventions that can be managed at home.

Main duties of the job

The Senior Staff Nurse will work within the Intermediate Care (Hospital@Home) Service.

The post holder will support the Team Leader in the future development of the Intermediate Care (Hospital@Home) Service. This will also include the facilitation of integrated working with colleagues in Local Authorities and 3rd sector organisations to support individuals within their home.

The post holder will also contribute to the development and implementation of policies and procedures within their sphere of responsibility and where appropriate, will actively participate in training and education programmes, patient support groups, research, clinical audit, and service evaluation.

The post holder will be responsible for providing support to the team leader in the day-to-day management of the service. Assisting the team leader in the management and requirements of the nursing team by utilising effective management and leadership skills.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Qualifications and Knowledge

Essential criteria
  • Registered Nurse.
  • Post Registration qualification to postgraduate Diploma level or skills, knowledge, and experience to equivalency.
  • Evidence of post registration continuing professional development.
  • Extensive knowledge of Speciality.
  • Working knowledge of multi-disciplinary teams.
  • Working knowledge of clinical governance and its application in practice.
  • Knowledge of audit and research.
  • Knowledge of Safeguarding and Adult Protection policies and procedures.
  • Knowledge of Infection Prevention and Control procedures.
  • Evidence of recent continues professional development.
Desirable criteria
  • Mentorship qualification or training.
  • Clinical Leadership qualification and/or training.
  • Knowledge of principles information governance.
  • Teaching certificate.
  • Local knowledge of statutory and voluntary service.
  • IQT – Bronze and Silver Practitioner.

Experience

Essential criteria
  • Previous experience of managing staff and managing resources (either as a team leader or a deputy).
  • Experience of undertaking clinical audit and/or research in clinical practice.
  • Experience in frail elderly, chronic disease management.
  • Significant experience as a community registered nurse.
  • Experience of implementing innovation and change in clinical practice.
  • Experience of interagency working and initiating developments across organisational boundaries.
  • Substantial post registration experience, most recently in a multi-disciplinary assessment service for those with complex needs (Intermediate Care or equivalent).
Desirable criteria
  • Experience of working within Primary Care and/or community care settings.
