Job overview

The post holder will be responsible for the coordination and provision of an effective assessment and treatment service to all individuals who find themselves detained in custody whether on remand or convicted.

You will work as a senior member within the Prison In-Reach multi-disciplinary teams working closely with members of the prison health care teams and be  responsible for the continuing development of the service , in conjunction with the Low Secure and Prison In-Reach Service Manager. 

The post holder will be expected to meet the complex needs of the identified client group, be responsible for liaison with key agencies (statutory and Voluntary) within the prison community and the wider geographical community.

This post is a fxed term/secondment for 10 months to cover maternity leave. If you are interested in applying for it as a secondment, please obtain permission from your current line manager prior to applying.

This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. Main duties of the job

Work as part of a multi-disciplinary, multi-agency team fostering good communication with colleagues of all disciplines and adhere to the Values set out by CTMUHB.

 Provide / receive complex,  sensitive and contentious information within a potentially hostile, antagonistic or highly emotive atmosphere in a secure setting.   

Be aware of the importance of maintaining confidentiality and security of information gained during the course of their duties.  

Ensure smooth transfer of care to specialist/out of area placements with appropriate local and national agencies

Attend weekly Single Point of Access Meetings (SPOA)  participate in discussions in relation to in-reach referrals and at times will be required to attend external meetings.

Work collaboratively with other agencies including G4S to effectively manage individuals, information gathering and contributing to case conferences and ACCT reviews .

Care coordinate/co work  individuals allocated to  your caseload 

Ensure record keeping meets the standard required by the NMC, IT systems as directed by the Senior Nurse/Team Leader and ensuring information sharing protocols are understood, and appropriately explained to clients.

Detailed job description and main responsibilities

In meeting the complex needs of the identified client group, be responsible for liaison with key agencies (statutory and Voluntary) within the prison community and the wider geographical community.

Key to the success of the post will be the effective working and partnership arrangements.  They will be representative at relevant groups, meetings, forums and to provide specialist advice, knowledge on the management of mentally disordered offenders.

You may be required at times to provide training to G4S staff around mental health disorders and management of behaviours.

Undertake comprehensive assessments using the Mental health measures framework, looking at strengths needs and risks (WARRN) as directed by the Team Leader.

You will be required to undertake a 5 day course ran by G4S prior to being allocated Keys to independently access the prison.

Qualifications and Knowledge

Essential criteria
  • Registered Mental Health Nurse. Professional knowledge acquired through Degree supplemented by post graduate Diploma specialist training. Evidence of continued professional development.
  • Good understanding of risk assessment
Desirable criteria
  • Welsh Language Skills (Level 3 and above/B1) are Desirable for this Role.

Experience

Essential criteria
  • Substantial post-registration experience in adult mental health care. Clinically credible with a background in multi-disciplinary working. Experience of managing/supervising staff.
  • Experience of working autonomously and using own initiative.
  • Experience of working within a multi-disciplinary team, and/or collaboratively with other organisations/agencies.
Desirable criteria
  • Experience of working in NHS Primary Care Services/mental health settings/care homes/in-patient services.
