Primary Care Nurse for Chronic Conditions

Band 6

Rydyn ni’n darparu gwasanaethau gofal iechyd i bobl sy’n byw ym Mhen-y-bont ar Ogwr, Merthyr Tudful a Rhondda Cynon Taf, ac yn gwasanaethu poblogaeth o 450,000. Mae ein lleoliad yn ddelfrydol; rydyn ni’n agos i brifddinas Cymru, mae Porthcawl i’r gorllewin ac mae golygfeydd godidog Bannau Brycheiniog.

Ein gweledigaeth yw gofalu am ein cymunedau a’n cleifion trwy atal salwch, hyrwyddo iechyd gwell, darparu gwasanaethau gwych a lleihau’r angen am ofal cleifion mewnol lle bynnag y bo’n bosibl, trwy ddarparu gwell gofal yn y cartref, gwell gofal sylfaenol a gwell gofal yn y gymuned.

Rydyn ni’n gwerthfawrogi amrywiaeth ein staff, ac rydyn ni’n croesawu ceisiadau gan bobl o grwpiau sy’n cael eu gwarchod gan Ddeddf Cydraddoldeb 2010; mae hyn yn cynnwys oedran, rhyw, cyfeiriadedd rhywiol, hunaniaeth rhyw/ailbennu rhyw, hil/cenedligrwydd, crefydd/cred, anabledd, beichiogrwydd a mamolaeth, priodas a phartneriaeth sifil.

Gwiriwch eich cyfrif e-bost yn rheolaidd. Bydd ymgeiswyr llwyddiannus yn derbyn eu holl ohebiaeth am y broses recriwtio trwy’r cyfrif e-bost gafodd ei nodi ar y ffurflen gais.

Mae croeso i bob ymgeisydd ymgeisio yn y Gymraeg neu yn y Saesneg, ac ni fydd ceisiadau yn y naill iaith na’r llall yn cael eu trin yn fwy neu’n llai ffafriol.

Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau

Trosolwg o'r swydd

An exciting and rewarding opportunity has arisen for a suitably qualified, enthusiastic and passionate Primary Care Nurse to join the Bridgend East Cluster.

The East Cluster includes five GP surgeries, covering a population of approximately 83,689 patients and works collaboratively alongside other Primary Care Contractors such as community pharmacies, Dental and Optometry colleagues, District Nurses, the Third Sector, other Allied health care Professionals and the Local Authority to support the health of our population. The cluster is fully committed to meeting the health needs of the population it serves and by working collaboratively with a ‘multi-disciplinary team’ approach allows us to achieve our goals.

The Primary Care Nurse will work within the cluster multi-disciplinary team, providing a domiciliary service for the co-ordination, management and monitoring of the care of the housebound patients with long-term chronic illnesses (heart disease, COPD and diabetes).

The emphasis of this role will be on service quality, through the optimisation of care where multi-morbidity exists, with the aim of maximising wellbeing, self-empowerment, quality of life and mobility.

If you are dynamic, organised, work well within the multidisciplinary team and have an interest in developing a career in Primary Care, would like to hear from you. An induction programme and development/training opportunities will be offered.
