Job overview
It is an exciting time for therapies within Cwm Taf Morgannwg. We are seeking a dynamic, motivated and innovative Physiotherapist to join our integrated multi-professional, national award winning service.
The Physiotherapist within the Stay Well At Home Team works closely with the Occupational Therapists, Social Workers and Support Workers to provide a rapid response service to people attending the Emergency Care Department, Acute Medical Unit and medical wards. The overall aim of the service is to avoid unnecessary hospital admissions and supporting timely discharges using a discharge to assess model.
The post holder will be a skilled and experienced practitioner and will be able to demonstrate a high level of clinical expertise and experience of supervising staff/students. Acting within their professional boundaries, the post holder will provide support and intervention for the presenting person through holistic person centred assessment, clinical assessment, diagnosis, treatment and evaluation of their care within set parameters and across the multi-agency team approach. They will demonstrate safe, clinical decision -making and expert care and support for people within the Care of the Elderly setting.

The post offers opportunities to enthusiastic and motivated staff to develop and share their skills. The service supports involvement in audit clinical supervision, in-service training, and continuing professional development with emphasis placed on regular supervision and support from experienced colleagues and from our Band 7 Support Group.

The Clinical Specialist Physiotherapist within the Stay Well @ Home Team is responsible for the early assessment of patients referred from A&E and the Acute Medical unit in order to prevent inappropriate admissions and enable timely discharge from hospital.
They will use highlight specialised, advanced clinical reasoning skills to determine a differential diagnosis whilst taking a major role in the advance assessment of falls risks, identifying further interventions required.
They act as an autonomous practitioner, providing expert assessment of the frail older person whilst also providing clinical leadership and operational management for the work of the band 6 physiotherapist and support staff (Across Stay Well @ Home) on the Acute Admission unit. During the absence of the corresponding Occupational Therapist and Principle Social Worker in the team, this post holder will also provide leadership and operational management to the social work and therapy component of the service
Person specification
Qualifications and/or knowledge
Essential criteria
- Degree/Diploma in Physiotherapy.
- HCPC Registration.
- Evidence of relevant, specialised post graduate courses/CPD to masters level equivalent.
- Knowledge of appropriate clinical guidelines – Local and National.
Desirable criteria
- Teaching qualification.
Experience
Essential criteria
- Extensive experience working within the care of the elderly field with particular reference to Falls.
- Experience of audit/research (clinical and organisational).
- Experience of formal and informal teaching.
- Experience of working within healthcare systems.
Desirable criteria
- Experience of working alongside Consultant Physician or similar.
- Experience of teaching at post graduate level.
Aptitude and Abilities/ Skills
Essential criteria
- Excellent communication skills (verbal, nonverbal and written).
- Ability to use highly specialised clinical reasoning skills to determine differential diagnosis, independent of other Practitioners, in an expert area of practice.
- Advanced assessment skills.
- The physical ability to perform and cope with all aspects of manual handling, equipment and advanced manual treatment techniques.
- Ability to work efficiently and effectively within a team environment.
- Coaching/mentoring/ presentation/instructing/class taking skills.
- Good IT skills.
Desirable criteria
- Welsh language Speaking/Listening Skills at Level 3 or above
Personal Qualities
Essential criteria
- Self motivated and capable of motivating others.
- Able to set priorities and deliver against targets.
- Ability to work collaboratively within the multi-disciplinary team.
