Job overview
The post holder will be responsible for the leadership and development of specific Community Services: this will include a diverse range of community services and related functions; supporting the effective working across the interface of community teams, clusters and secondary care, social care and 3rd sector.
The role will oversee the new Hospital@home service and community discharge teams.
As a key member of the Primary and Community Care Group leadership team the key focus will be supporting the operational delivery with the team managers and clinical leaders, embedding the community transformation agenda and new ways of working as business as usual. Focused on safe, effective and person- centred care, you will lead the delivery of integrated community services . The post holder will take the lead in the development of efficient pathways to support patients across the wider system.
The post holder will be a person focused leader who has a proven track record of delivering high quality services, leading by example, show a commitment to team working and driven to supporting and developing staff.
Main duties of the job
Managerial responsibility for the effective operationalisation of the community transformation plans, ensuring delivery of the organisational quality, performance and financial targets in line with the Care Group and Health Boards aspirations.
Work with the Senior Management Team to ensure the strategic direction, as defined within the Health Board is effectively implemented at an operational level. for the Hospital@Home and Discharge Service.
The post holder is a member of the Senior Management Team and as such has responsibility to contribute to the re-design of services and to participate in the direct management of services and provide sound advice and guidance to the Directorate Manager and wider Leadership Team.
Work collaboratively with the other Integrated teams and professionals across the Locality and Health Board footprint to ensure consistency of approach and roll out of best practice, and seamless pathways of care.
Ensure that there is a clear accessible referral system and pathway into and within the service to relevant care providers, and to monitor its effectiveness through the appropriate performance framework.
Monitor and maintain high standards of health and care service delivery, and to provide innovative solutions to improving service quality.
Regularly monitor and review the service plans and adjust for changes in local need and in response to changes in national policy guidance.

Detailed job description and main responsibilities
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. Undertake the role of Deputy Locality Manager and ensure the effective leadership of community teams ensuring they own and deliver the standards of quality and performance expected of them.
Have line management responsibility for several services within the Community undertaking recruitment, appraisal, disciplinary, investigations, grievance etc., as required that exposes the post holder to emotional and distressing circumstances and will need to work in accordance with CTMUHB policies.
Develop robust partnerships with trade union/staff side organisations across CTMUHB.
Lead and motivate the Community teams to ensure a high standard of professionalism, efficiency, effectiveness and innovation in the delivery of placed based person-centred care and support.
Provide leadership and direction across situations where highly complex ideas or concepts need to be conveyed and implemented across the service in easily understood language.
Ensure that effective communication mechanisms are in place within the team to engage staff and staff organisations, recognising the particular challenges associated with a dispersed workforce.
Lead the Community teams to promote and ensure a culture of continuous improvement and professional excellence is achieved, encouraging the sharing of best practice across the organisation.
Ensure the provision of clear objectives to managers within the Community Services ensuring individual development plans are progressed and that effective systems of joint professional/clinical supervision are in place.

Qualifications/Knowledge
Essential criteria
- Master’s Level qualification or equivalent relevant skills, knowledge and experience
- Knowledge and understanding of the political context and constraints at local and national level.
- Comprehensive knowledge and understanding of governance principles and systems
Desirable criteria
- Postgraduate management qualification
Experience
Essential criteria
- Experience of operational management, including Staff, Performance, Service Development and significant financial management.
- Experience in contributing to plans and policies at a strategic level
- Experience of working effectively with partners, stakeholders and the public to deliver change
- Experience in establishing and leading effective teams.
Desirable criteria
- Experience of operating in a political environment and developing relationships with local elected members.
