Job overview

An excellent opportunity has arisen for an Advanced Clinical Practitioner to join our well established Frailty/Care of Elderly Team. The role will require the successful clinician to work within the four pillars of Advanced Practice which includes:

 Clinical management of patients ,lead on service developments and change. Provide education mentorship and support to a wide multi-professional audience and demonstrate understanding and use of evidence based practice and involvement in research (QIP/Audit).

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. Main duties of the job

The ACP will be expected to provide clinical care/management to groups of patients admitted to the  Frailty/Care of Elderly service. 

Lead on service development. Provide leadership to MDT members.

Education -deliver educational packages to a multi-professional audience. Maintain own level of education

Research - be involved in relevant research/audit/SE/QiP's

Detailed job description and main responsibilities

The ACP will be expected to provide clinical care/management to groups of patients admitted in the Frailty/Care of Elderly service. The service currently supports  general wards and dementia and delirium wards within the Community Hospital 

Clinical:

They will be expected to undertake clinical assessment request relevant investigations, make diagnosis and treat and manage patients according to need. This would include non-medical independent prescribing. They will be required to undertake regular patient ward rounds as an independent clinician and support patients through their hospital journey from admission to discharge. They will be expected to support effective communication between ward teams, MDT and patient/relatives. Complex discharge processes and lead on ACP led discharges. To undertake higher level communication- impart bad news and discuss treatment escalation plans with patients and carers. 

Leadership – clinical leadership of MDT. Develop service. Develop projects. Attend and be an instrumental part of relevant HB strategy 

Education- be responsible to own education needs. Maintain a live portfolio of AP evidence. Deliver education within the clinical setting and in more formal education settings. Provide mentorship and coaching to multi-professionals undertaking clinical courses such as MSc AP and NMIP.

Research- use guidelines to support clinical decisions in practice. Be involved in audit/research. Lead of QiP projects.

The job is open to fully trained ACPs from multi-professional backgrounds.
