An exciting and rewarding opportunity has arisen for a suitably qualified, enthusiastic and dynamic Advanced  Nurse  Practitioner, to join our well-established community MDT within the @Home Service, based at Dewi Sant Health Park. 

We welcome applications from those individuals in receipt of a Level 7 education ( MSc ) relevant to area of practice or equivalent  experience. Also in receipt of an independent prescribing qualification. 

The post holder will work closely with the team and provide effective leadership and management to ensure the delivery of safe, effective, well led, responsive, quality patient care.

Alongside this they will need to be able to demonstrate the ability to contribute to the development and implementation of policies and procedures within their sphere of responsibility and where appropriate, will actively initiate and partake in training and education programmes, research, clinical audit and service evaluation, demonstrating their ability to practice autonomously across all four pillars of advanced practice.

You should be passionate about improving the patient experience and quality of clinical outcomes and possess good organizational, interpersonal and communication skills.

Main duties of the job

The post holder is responsible for the holistic assessment of care needs and development implementation and evaluation of evidence based programmes of care.  The role will specifically focus on expert clinical practice, education skills.

As an Advanced Nurse  Practitioner, the post holder will be expected to exercise a high degree of professional autonomy in relation to patients nursing needs and to satisfy the expectations and demands of the role.

The post holder will assist the Lead Advanced Nurse Practitioner in the clinical assessment of patients who have a complex and undetermined presentation working alongside medical and GP colleagues to maintain patients in a community setting and avoid hospital admissions and facilitate discharges.

The Advanced Nurse   Practitioner will be expected to work with Health and Social Care colleagues in a number of key areas which include reablement, avoidance of hospital admission and  facilitation of early discharge.

The post holder will be expected to have a full understanding of the relevant care pathways and be able to work with Health and Social Care staff on work streams arising out of the transition of services currently being evolved.

The post holder will be expected to act on their own initiative to differentiate appropriately between those issues which need to be referred to others for a decision from the issues upon which the post holder can exercise appropriate decision making.

Qualifications and/or Knowledge

Essential criteria
  • MSc Advanced Practice - Level 7
  • Independent prescribing
  • Extensive experience of working as ANP band 7
  • Holds a live Advanced Pracititoner portfolio of evidence
  • Confident and competent in a wide range of advanced clinical skills
  • Experienced in chronic disease management
Desirable criteria
  • Teaching qualification
  • Experience of developing and leading on programme of education

  • Excellent leadership skills, ability to implement change in response to local/national developments/ research
  • Analytical and judgemental skills enhancing the decision making process to resolve complex issues /situations

  • Experience of service development
