Main duties of the job

The successful candidate will provide professional and operational management to pharmacy technicians working within the Intermediate Care (Hospital @ Home) service.  They will play a key role in the development, co-ordination and delivery of medicines management support by the Hospital@Home service to patients in the community and following their discharge from hospital.

In addition to overseeing a team of pharmacy technicians, the post holder will assess patients’ ability to manage their medication and support them with tailored care plans.

The senior technician will work alongside clinical pharmacists and other healthcare professionals within a newly established multi-disciplanary team.  Coordinating  with hospitals, GPs, community pharmacies and social care providers, the post holder will be integral to  the delivery of safe and effective care. 

Working independently but within a supportive team, the role will utilise clinical skills and evidence-based approaches to improve patient outcomes.

Detailed job description and main responsibilities

Act as a key contact and advisor within the Intermediate Care (Hospital@Home) Service on issues related to medicines management, including those raised by partners within primary, secondary care and social care.


Lead and co-ordinate work across Health and Social Care to improve medicines management, promote independence and improve care outcomes for people requiring support in their own homes.

Oversee and undertake individual assessments of patients in their own home following discharge from hospital and identify their medicines management needs.

Ensure effective liaison with colleagues across health and social care to establish a suitable level of assistance, supporting more generic staff in the delivery of this assistance.

Qualifications

Essential criteria
  • NVQ3, BTEC in Pharmaceutical Sciences or equivalent
  • Registered with the GPhC
Desirable criteria
  • Accredited Checking Technician
  • Medicines Management Technician
  • Quality Improvement qualification
  • Leadership / service development qualification

Knowledge

Essential criteria
  • Knowledge of medicines management issues re primary & secondary care
  • Knowledge of the Social Services and Wellbeing Act
  • Knowledge and understanding of available systems to support patients in the community
  • Knowledge of prescribing and pharmaceutical systems and processes across care settings
Desirable criteria
  • Understanding of the health/social care interface where it relates to medicines management.
  • Understanding of quality improvement and IQT methodology.

Experience

Essential criteria
  • Significant post qualification experience as a pharmacy technician.
  • Medicines management/near patient pharmacy services.
  • Working within a multi-disciplinary team.
  • Experience of service development/improvement.
Desirable criteria
  • Previous experience of conducting medication needs assessments
  • Experience of supervising staff.
  • Experience of implementing new procedures or services.
  • Experience of medication accuracy checking and reconciliation.

Skills

Essential criteria
  • Effective verbal, written and interpersonal communication skills.
  • Ability to manage conflict and deal with difficult situations.
  • Effective organisational and prioritising skills
  • Computer literate including report writing skills.
Desirable criteria
  • Welsh speaking/listening at level 3 or above
