Job overview
This is a brand new post within the Health Community Support Team, for the Health Board.
This service will provide intermediate care, support and rapid access to mutli professional intervention in order to support patients from discharge from hospital and avoid unneccessary admissions into hospital.
You will be operationally managing the AHP resource within this service, which will work across the Health Board.
Main duties of the job
- The post holder is identified as a highly skilled, senior practitioner with expert knowledge in Community and Intermediate Care.
- The post holder will provide strategic and operational leadership for AHP’s working within the service.
- The post holder will work with the community, primary care and admission avoidance teams across the health board and local authorities to respond in a timely way to urgent patients.
- The post holder will take a lead on the continuing development of the Intermediate Care Service and its strategic and operational planning.
- The post holder will undertake research and development on an ongoing basis, leading clinical audit and research associated with the Intermediate Care Service.

Qualifications
Essential criteria
- Degree/Diploma in Occupational Therapy, Dietetics, Speech and Language Therapy or Physiotherapy
- Post graduate Diploma level in relevant area.
- Professional registration as recognised by HCPC. Evidence of attendance on relevant short courses.
- Knowledge of Health and Social Care legislation. Knowledge of current health and social care policies and application in clinical area.
- Leadership/Management qualification or evidence of management development e.g. leadership/coaching/mentoring.
- Evidence of knowledge/experience or qualification in research and audit. Knowledge of the requirements of clinical governance.
- Knowledge of Health Issues affecting the local population and strategies in place to support improvement in these areas.
- Evidence of professional development. Fieldwork education training qualification.
- Understanding of professional ethics and their application in practice.
Desirable criteria
- Member of professional body. Member of relevant specialist interest group.
- Project/programme development experience.
- Knowledge of current agenda for health and social care provision.
- Knowledge and understanding of the impact of health and socio-economic factors on the health of the local population.

- Extensive post graduate experience. Extensive experience of operational management and supervision of staff

- Experience of working within an intermediate care setting. Direct involvement with carers and families.
- Negotiation of service development. Evidence of supporting service compliance with clinical governance requirements.
- Experience of appropriate delegation and supervision. Evidence of student supervision. Extensive practical experience of human resource management, operational management and clinical/operational supervision.
- Experience of developing service, clinical and professional standards and policies. Experience of planning and delivering formal and informal teaching.
