Job overview
In CTM, we’re building our services to make a real difference — and you could be part of it. Our @Home service is based at Dewi Sant Health Park and across RCT and Merthyr, and we are looking for a Band 6 Occupational Therapist to join us.
This is more than just a job. It’s an opportunity to be part of something fresh, meaningful, and transformative. Together, we’ll work to reduce unnecessary hospital admissions and help people live more independently in their own homes.
As an Occupational Therapist, you’ll play a key role in shaping how we deliver care. You’ll be assessing and supporting individuals in community settings, including their homes, and linking closely with integrated Health & Social Care teams.
We’re looking for someone who’s:
- Open-minded and adaptable — ready to grow with the service as it evolves.
- Passionate to deliver care closer at home, and empower people to live independently.
- A team player who thrives in a dynamic, collaborative environment.
Main duties of the job
This is your chance to help shape a service that’s making a real difference — and to grow professionally in a supportive, innovative environment. As part of the @Home team, you will:
- Work autonomously across community settings, undertaking lone working in service users’ homes
- Manage a defined caseload, delivering specialist assessment and intermediate care
- Apply evidence-based, person-centred approaches to plan and evaluate interventions
- Maintain accurate clinical records in line with professional and local standards
- Develop your skills and expertise through a personal development plan
- Provide leadership and supervision to new staff and students
- Collaborate with a wide multidisciplinary network including hospital teams, care managers, third sector organisations, and primary care
Your impact will be seen as admission avoidance and reduction in hospital stays, while improving independence at home.
This is a full-time role, working Monday to Friday, 8am -4pm ,offering a consistent schedule while making a real impact to support your patients to live as independently as possible.
