Job overview
We are seeking a highly motivated and compassionate Band 5 Community Staff Nurse to join our dynamic and supportive District Nursing team. This is an exciting opportunity to provide high-quality, patient-centred nursing care within the homes of individuals across CTMUHB.
As a key member of our team, you will be responsible for assessing, planning, implementing, and evaluating holistic nursing care for a diverse caseload of patients, promoting independence and well-being. You will work autonomously, managing your own caseload, whilst also collaborating closely with a multidisciplinary team including GPs, social workers, therapists, and other healthcare professionals.
This role requires excellent clinical skills, strong communication abilities, and a commitment to delivering empathetic care within the community setting. Main duties of the job
A Band 5 Community Staff Nurse provides holistic, patient-centred care in homes. Duties include: comprehensive assessment and individualised care planning; diverse clinical interventions like wound care, medication administration, catheter care, and palliative support; autonomous caseload management, prioritising visits and documenting accurately. They communicate effectively with patients, families, and multidisciplinary teams (GPs, OTs, Social Workers) for integrated care. The role demands strong problem-solving, health promotion, safeguarding awareness, and continuous professional development, ensuring safe, effective, and compassionate nursing care in the community.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing candidates. This isn’t ‘fluency’, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. - Autonomous Practice and Time Management; Assessment and Care Planning: Ability to conduct comprehensive patient assessments, develop, implement, and evaluate individualized care plans in collaboration with the patient and other healthcare professionals.
Desirable criteria
- Experience with Specific Conditions/Client Groups: Experience in managing patients with long-term conditions (e.g., diabetes, COPD, heart failure) or specific client groups relevant to community nursing (e.g., palliative care, continence).
- Mentorship/Teaching Experience: Experience in supervising or mentoring junior staff, student nurses, or healthcare assistants, or a willingness to undertake mentorship training.
Skills
Essential criteria
- Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to effectively liaise with patients, families, carers, and a multidisciplinary team. Proven ability to provide empathetic and patient-centred care.
- Autonomous Practice and Time Management: Ability to work autonomously, manage a defined caseload, and prioritise workload effectively in a dynamic community setting. Must demonstrate good organisational skills and the ability to work independently.
Desirable criteria
- IT Proficiency: Competence in using electronic patient record systems (EPR) and other relevant IT systems for documentation, communication, and accessing patient information.
- Welsh language Speaking/Listening Skills at Level 3 or above
Knowledge
Essential criteria
- Evidence of Continuing Professional Development (CPD): Participation in recent professional development activities relevant to community nursing, such as study days, short courses, or portfolio development.
Desirable criteria
- Understanding of Community Nursing Frameworks: Knowledge of relevant policies, guidelines, and frameworks governing community nursing practice, such as safeguarding, mental capacity act, and continuing healthcare.
Other

- Ability to work autonomously, manage a defined caseload, and prioritise workload effectively in a dynamic community setting. Must demonstrate good organisational skills and the ability to work independently.
