Job overview

An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the Newbridge District Nursing Team on a permanent contract for 24hrs hours, which would include some weekend working. 

The Newbridge  team provide clinical care and interventions for patients in their own home under the medical governance of Primary Care GP's.    

The District Nursing/Community team in Newbridge  works as a partner within a wider multi-disciplinary team which includes CRT Frailty Consultant, General Practitioners,  Care at home, Physiotherapists, Occupational Therapists, Social Workers, Support and Wellbeing workers and the Voluntary Sector.

An informal visit by anyone prior to application would be welcomed by the team.

An exciting opportunity has arisen for a suitably qualified Band 4 Assistant Practitioner to join the Newbridge community team in the provision of clinical care to individuals in their own home.

As Assistant Practitioner you will work with the wider community MDT under the delegation of your Band 7, Band 6  and a team of Registered Nurses.  You will utilise and develop a wide variety of skills to meet the emotional, physiological, and clinical needs of individuals.

The post holder will work collaboratively and support members of the multidisciplinary team in the delivery, development and management of  health and appropriate referral for social care issues.  The role is challenging but rewarding and you will be fully encouraged and supported with your professional development. 

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

This post is permanent, part time, and can include weekends.

Essential criteria
  • Experienced in undertaking a range of clinical skills relevant to the post as per role scope of practice and the national governance framework
  • Significant and recent experience of providing relevant clinical care
Desirable criteria
  • For community roles: experience of providing care in a community or primary care setting.

Qualifications

Essential criteria
  • Level 4 vocational qualification in a relevant health care associated subject / Certificate in Health Care Support Worker Education
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
Desirable criteria
  • Ability to speak or learn Welsh
  • Trained to perform venepuncture
