Job overview

The post advertised has become available as a replacement into an existing consultant complement. This is a 10 session post, providing senior medical input into community care for the Forensic service. The post holder will be supported by secretarial and administrative staff and will have use of appropriate office space and Information Technology resource. You will also be supported by a Full Time Specialty Doctor

The post holder will join a forward thinking, motivated and enthusiastic Forensic Team, working across a pan Gwent community forensics team. The service is undergoing positive change, which is actively shaping secure care services. In addition to the team, we have strong links between the members of the secondary care services including acute wards and CMHTs, multi-disciplinary/multi-agency team, GP’s and voluntary agencies.

The following list indicates the initiatives currently high on the strategic agenda for the forensic service:

a)        Investment in local rehabilitation care pathways to reduce readmission rates and manage risks through transition periods.

b)       Establishing full multi-disciplinary working across service areas to ensure robust active care and treatment planning, along with diversion and prevention of harm.

c)        Establishing a clear and meaningful forum for co-production of services.

Main duties of the job

The post holder will take the lead in providing a service to adults within the forensic service, alongside consultant and specialist psychiatrist colleagues. The forensic mental health service sector operates a traditional service model leading the community service within Gwent and leading the inpatient care for sector patients admitted to the rehabilitation units (Pillmawr and Ty Skirrid). These wards are covered by a full-time rehabilitation consultant psychiatrist.

The post holder will conduct outpatient clinics (typically in the form of home visits) within the community including CTP review/117 review meetings.  They will hold RC responsibility for a number of restricted patients, which will require regular liaison with the ministry of Justice.  As an integrated member of the team, they will foster close working relationships with other members of the community team and with partner organisations.  The post-holder will assume RC responsibility for any MOJ patients who are admitted to acute, adult mental health wards.

Cover for study leave, annual leave and sick leave is provided by other medical staff covering the forensic/rehabilitation team, and this would be expected to be reciprocated.
