The main duties of the posts are to care for individuals with complex health care needs in their own homes within the community. 

Main duties of the job

Are you someone who has an interest working with individuals with complex health needs who wants an exciting new challenge?

 

Vibrant, dynamic and suitably experienced Health Care Support Worker required to care for a young male, living in the Torfaen Area.

HCSWS should have knowledge and experience of working with patients with complex health needs in the community. HCSW’s are welcomed who would be willing to learn these skills. This role would also include rotation shifts to meet the needs of the service and ensure skill competency is compliant.

Successful staff will be expected to enable the individual to live a full and productive life supporting physical and psychosocial needs, inclusive of using their vehicles in the community to attend appointments and social activities. This unique opportunity allows the HCSW to develop a relationship with the patient to get to know his individual needs and his health needs to provide high, quality skilled and individualised care.

There is developing, exciting work within the Care at Home Team within ABUHB. The service is driven towards succession planning and there are a variety of training and development opportunities available for HCSW’s within this unique service.

Full UK driving licence is essential for this post

Recent NHS or Care Home experience within the last 12 months is essential.

A proven track record of being reliable and dependable.

A flexible approach to work would be a great advantage.

Qualifications

Essential criteria
  • Level 3 qualification in a health care related subject Or • Equivalent experience and can evidence work based competency demonstrating theoretical knowledge, and the ability to undertake clinical care duties relevant for the role which is gained through experience and worked based training/short courses.
  • Knowledge of relevant policies and procedures within a healthcare setting for example the principles of Safeguarding adults/children to keep patients within their care safe, and confidentiality for example General Data Protection Regulations (GDPR) .

Experience

Essential criteria
  • Experience of working within a relevant heath or social care environment, providing direct patient care.
Desirable criteria
  • Experience of working with patient groups relevant to area of practice.
  • Experience of working within the NHS

Skills

Essential criteria
  • Physically able to carry out the duties of the role, including lifting, assisting with mobility, and performing other manual tasks.
Desirable criteria
  • Full UK driving Licence
  • Ability to travel within geographical area to meet the needs of the service
