Job overview
We are seeking a number experienced and forward-thinking General Practitioners to take on a GP Interface Lead role, supporting the integration of primary and secondary care across Aneurin Bevan UHB. These roles are pivotal in improving patient pathways, reducing unwarranted variation, and enhancing clinical collaboration between sectors.
You will act as a clinical interface, ensuring referrals are appropriate, timely, and aligned with best practice. Working closely with secondary care Consultants and operational teams, you will help shape a more efficient and patient-centred system, supported by a robust clinical governance framework.

Main duties of the job
The Interface GP role is a vital role in the delivery of our ambitions to achieve the best possible health outcomes for the people of Aneurin Bevan University Health Board. The Interface GP will improve collaboration between Primary and Secondary Care, promoting mutual learning, seeking challenge and continuously driving innovation and improvement.
The Interface GP will work as part of our multidisciplinary teams and support the delivery of effective coworking and pathway integration through;

Triage and management of Primary Care referrals to Secondary Care,

Improving communication and the transfer of learning between Primary and Secondary Care
