Job overview
To support B7 in the provision of effective, efficient and appropriate high quality nursing care for patients who have experienced falls in the community or in the hospital setting. To manage fragility fracture (explaining diagnoses, sharing treatment options and monitoring compliance to treatment and its side effects)
Assess, treat and support patients who have sustained falls or are living with the impact of falls, particularly fragility fractures
Promote evidence-based practice and act as an accountable role model
Promote patient pathway, ensuring effective and timely interventions
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The post holder will undertake comprehensive nursing assessments for patients with falls and osteoporosis, which may include physical examinations and comprehensive patient histories; accessing previous radiological investigations, GP letters and clinic letters.
Assess and manage risk within the care environment.
Refer patients for relevant investigations and tests required to assist other clinicians in diagnosis
In collaboration with the wider team implement therapeutic interventions and referrals to others as appropriate.
