Job overview
- Deliver high-quality, person-centred nursing care in the community during periods of acute illness.
- Undertake and contribute to Comprehensive Geriatric Assessments to support frail individuals in remaining at home.
- Facilitate early discharge from hospital by working closely with the Grange Front Door Team and community services.
- Provide timely nursing assessment, care planning, implementation, and evaluation.
- Promote health and prevent illness through proactive interventions and education.
Main duties of the job
- To be responsible for the assessment, planning implementation and evaluation of care for community-based patients.
- To work as a member of the wider primary care and community multi-disciplinary team.
- Administration of IV therapies within the community and Cedar Unit Clinic.
- support the patients at front door at grange to be pulled back into community- with knowledge of community offer.
- To support the Team Leader, in developing the Rapid Response Team and raising the profile of the Rapid Response Service within the Borough.

Experience
Essential criteria
- Significant experience working as a qualified RN.
- Assessment and provision of care to people within frailty/community.
- Working as part of a multi disciplinary team.
- Mentoring Students and other Healthcare professionals.
Desirable criteria
- Involvement of undertaking a training needs assessment and analysis.
- Staff caseload supervision.
- Team Leading.
- Supervision and development of junior staff.
- Staff reviews/appraisals working with staff to identify development needs.
Qualifications
Essential criteria
- Registered Level 1 Nurse, Degree Level Education
- Evidence of recent post registration, professional development
Desirable criteria
- CPA
- Post Grad qualification

Skills
Essential criteria
- Able to perform extended clinical skills; Intravenous therapy, cannulation, phlebotomy, syringe drivers, male and female catheterisation, removal of sutures
- An understanding of the structure for the delivery for primary health care services.
- Able to communicate succinctly and effectively verbally, in writing and electronically
- Able to present information effectively to individuals and groups
Desirable criteria
- ECDL or equivalent.
- Able to lead a team of nurses and support workers.
- Demonstrates ability to research, critique and change practices.
Knowledge
Essential criteria
- Knowledge and compliance with NMC Codes of Practice.
- An up to date working knowledge of :- Acute, chronic and terminal illness, wound management, care of the elderly, drug interactions, aetiology and physiology of pain, pain assessment, and strategies for effective pain and associated symptom control.
Desirable criteria
- Clinical Supervision
- Knowledge and understanding of audit and research
- knowledge of frailty and admission avoidance.
