Job overview
The Peter Sowerby Dementia Breakthrough Partnership was formed in 2019 to realise the full potential of GPs to transform dementia support; to improve the lives of millions of people with dementia, today and tomorrow, through primary care-led post-diagnostic support.
Alzheimer’s Society were very fortunate to be awarded funding from the Peter Sowerby Dementia Transformation Fund with the aim to transform the quality of life of people affected by dementia by empowering
primary care and community care providers to deliver high-quality support at the point of, and following, diagnosis.
In partnership with NSFT, we are aiming to deliver on the initiative by offering this post to a suitable applicant. We are looking for a Mental Health or Learning Disabilities Nurse who has suitable experience and enthusiasm for working with people who have memory and cognitive problems and who is passionate about improving the lives of the people who do. This is a wonderful opportunity to work across services with stakeholders.
Main duties of the job
- Use the practice records to identify people with memory concerns, including those who may be placed on an MCI pathway.
- Support Primary Care colleagues to have early conversations with people about Brain Health and Dementia risk factors, especially those in midlife living with conditions such as hypertension or Diabetes, for example, during Annual reviews or check-ups.
- Provide learning opportunities to up-skill primary care colleagues, including Social Prescribers, paramedics and care navigators.
- Review MCI patients and in doing so, help to embed this into the culture of Primary Care.
- Along with the Dementia Advisor, help to improve access to support and care pre-assessment, links to community support and psychoeducation, especially for those with an MCI.
- Contribute to weekly MDT/Clinical meetings already facilitated in the practice.

Essential criteria
- Mental health nurse, occupational therapist, social worker or equivalent.
Desirable criteria
- Further education in specialty area
Skills

- Reasonable confidence and competence in using IT systems (e.g. Outlook, Word, clinical or other record systems) relevant to role
Desirable criteria
- Able to demonstrate sound clinical expertise and knowledge of mental health disorders and their treatments.
Experience
Essential criteria
- Extensive experience of practice within clinical area with proven track record in delivering high quality care
Desirable criteria
- Experience of implementing evidence based practice and or service improvement initiative.
