Case Manager 

37.5 hours per week

The Complex Discharge Team is looking for an enthusiastic and motivated person to join our friendly team to cover Maternity leave on a 1 year secondment/fixed term contract. 

The successful candidate must have excellent communication skills and be able to confidently work with the ward Multi Disciplinary Teams and external services.

Case managers are an essential part of the flow of patients within the hospital, to manage a case load of patients to prepare them for safe and effective discharge planning.

You would be required to attend ward board rounds on a daily basis and to liaise with the Discharge Hub to highlight complex patients and those who require discharge planning.

Applicants should be able to use their own judgement to resolve issues and have a professional approach to managing often challenging situations.
