Job overview

An exciting secondment opportunity has arisen to join the Older Persons Community Mental Health Team in Great Yarmouth to cover a period of Maternity leave.

We are a friendly, supportive, and experienced multidisciplinary team committed to delivering high-quality, evidence-based, person-centred care to older adults, typically aged 75+, experiencing functional mental health difficulties and those of any age experiencing problems associated with Dementia.

This role is ideal for an experienced Band 6 practitioner or a Band 5 looking to progress into a more senior role . We are passionate about supporting staff to grow and will provide the guidance, supervision, and training needed to help you succeed in the role.

Previous experience of working with older adults is not essential as you will receive a  tailored induction programme.

Main duties of the job

The post holder will:

  • Work as a Care Coordinator for a caseload of service users with complex mental health needs
  • Deliver person-centred, evidence-based interventions to older people and their carers
  • Undertake comprehensive assessments, including risk formulation and safety planning
  • Liaise effectively with a range of professionals and services to support holistic care delivery
  • Participate in daily Safety Huddles and weekly MDT meetings
  • Contribute to the shared Duty Worker role within the team
  • Triage referrals into the service
  • Maintain accurate and timely clinical records using electronic systems
  • Support and mentor students and colleagues as appropriate

Caseloads are carefully managed using a caseload weighting toolkit to ensure they remain safe, balanced, and sustainable, supporting staff wellbeing.

Detailed job description and main responsibilities

Clinical Care 

  • Deliver high-quality, person-centred nursing care to older adults within the community, in line with agreed care plans.
  • Undertake initial,  and ongoing patient assessments, including risk assessment and screening.
  • Continuously review and evaluate patients’ needs, supporting safe and timely discharge planning in collaboration with senior colleagues.
  • Enable and support patients to maintain and maximise their independence wherever possible.
  • Promote the active involvement of patients and carers in care planning, ensuring they are informed, empowered, and central to decision-making.

Care Coordination and Team Working

  • Manage and prioritise a defined caseload, organising your workload effectively across the community setting.
  • Work collaboratively within a multi-disciplinary team, building strong partnerships with health, social care, and voluntary sector colleagues.
  • Initiate and contribute to reviews, case conferences, and other multi-agency meetings.
  • Communicate any changes in patients’ conditions or circumstances promptly to the wider team, including medical and social care professionals.

Leadership and Supervision

  • Provide guidance, support, and supervision to junior staff as delegated by the Senior Nurse.
  • Support the learning and development of colleagues and contribute to a positive team environment.
  • Act as a mentor for students and learners, supporting placements in line with professional standards.
  • Contribute, where appropriate, to recruitment and selection processes.

Professional Practice and Development

  • Maintain accurate, timely, and high-quality clinical records in line with Trust standards.
  • Ensure all practice is evidence-based and informed by current local and national guidance.
  • Take responsibility for your own professional development through appraisal, training, and reflective practice.
  • Participate in clinical governance, audit, research, and service improvement activities.
  • Work in accordance with the Mental Health Act (1983) and other relevant legislation and policies.

Safety, Quality, and Compliance

  • Ensure the safe handling, administration, and storage of medications and clinical materials.
  • Adhere to all Trust policies, procedures, and professional standards at all times.
  • Maintain professional, therapeutic relationships with patients, carers, and colleagues.
  • Keep the Clinical Nurse Leader informed of any clinical or service delivery concerns.
  • Use resources effectively and organise your working day to meet the needs of the service.

Education and Training

  • Actively participate in training and development opportunities.
  • Support and contribute to the learning environment for student nurses and other learners.
  • Maintain up-to-date mentorship skills and competencies.
