Senior Practitioner Band 6 role working in the Crisis Resolution Home Treatment Team

We are a unique team in that we are integrated to the ward and our day-care facilities as a partial hospitalisation service which enhances and supports patient centred care to provide a smooth transition throughout our services to ensure that the highest quality of care is received for all our patients and carers.
Main duties of the job
Within this role you will also carry out emergency assessments from Community Teams, GP's and A&E Out of Hours in view of gate-keeping possible admissions, ongoing care or referral to external teams. The team also visits Service Users at home to plan and implement care packages for Crisis and Home Treatment Team patients.

Detailed job description and main responsibilities
You will be able to work flexibly as this service covers 24 hours 7 days a week. Although you will be working as part of a multi-disciplinary team, there will be elements of lone working. The post will sit under the Great Yarmouth & Waveney Acute Mental Health Services within a supportive and progressive Locality.
