If you have answered yes then please apply for our exciting opportunity for an Advanced Nurse Practitioner to work within the Crisis and Resolution Home Treatment  Team, based at Hellesdon Hospital. 

Main duties of the job

The team will consist of a variety of highly skilled professionals including nursing, support staff, medical, psychology and occupational health colleagues. You will work closely with all of them to ensure the care we provide is not only outstanding but that the team is well developed and supported.

We would expect you to -

  • Act as a role model to what outstanding care should look like.
  • Support and mentor clinical staff to deliver person centred care.
  • Provide family and carers with assurance that their loved one is being supported and looked after.
  • Work within a supportive management team to improve patient journey through secondary care services. 
  • Undertake clinical audits and screenings
  • Provide Line Managed Supervision, including in depth caseload reviews.
  • Supporting safe and effective discharges.
  • Providing medication reviews.
  • Undertaking assessments as and when required.
  • Supporting with 72 hour follow up. 
  • Independently undertake complex prescribing reviews
  • Confidence in acute risk management
  • Provide clinical leadership to the team

Qualifications

Essential criteria
  • RN Mental Health or General Nurse
  • Post registration qualification in speciality Educated to Master’s degree level in relevant subject area
  • Recognised leadership qualification or equivalent experience
  • Independent Prescribing qualification

Skills

  • Experience of working in an acute setting
  • Experience in managing complaints and quality assurance
