The Cancer Care Coordinator plays a key role supporting patients and coordinating care throughout their cancer diagnosis and treatment pathways.
Main duties of the job
We are looking for someone enthusiastic and proactive to coordinate a busy service with large numbers of patients with varying care needs.
You will work closely with a small team of Clinical Nurse Specialists to support patients with a diagnosis of breast cancer.
Previous experience or ability in effective coordination and managing patient care are essential, and compassion and empathy are pivotal to this role.
You will need excellent communication skills with experience of working in a health care setting.

Detailed job description and main responsibilities
The role entails:
Triaging patient enquiries and providing follow up support to patients to ensure their care and recovery is taking place as planned.
Undertaking patient holistic needs assessments, and implementing responses in line with agreed pathways.
Providing administrative support to the Cancer Clinical Nurse Specialist team, ensuring activity is collated, patient pathways are tracked, supporting patient education programmes, collating patient experience and supporting audit. As such you should be able to demonstrate excellent IT literacy organisational and service management skills.
Meeting deadlines and targets with a flexible and adaptable approach to work.
Working closely with staff across the hospital and other service providers, you will need to have a professional manner and an excellent standard of communication. Attention to detail and the ability to work effectively as part of a multidisciplinary team is essential.

Essential criteria
- 3 GCSE qualification grade 9 to 4 equivalent including Math and English.
- Educated to A level/Level 3 qualification in Health or Social Care, Higher National Certificate or equivalent relevant experience
- Experience of working in a Healthcare setting without direct supervision
- Experience of working in the relevant clinical experience
- Experience of working with multi-professional teams
- Experience of working using own initiative and prioritising own workload
- Experience in participating in clinical supervision/ reflective practice
- Demonstrates an understanding of role applied for within the clinical context
- Excellent communication skills
Desirable criteria
- Evidence of previous learning related to clinical area to diploma level or foundation degree
- Experience of telephone triage and patient contact
- Administration support for a clinical team.
- Experience of participation of clinical audit
- Reflective practice skills
