Job overview

The post holder will be responsible for delivering and leading highly specialist care within the Community Care Team. In doing so they will work closely with partner organisations in supporting and overseeing the assessment, planning and delivery of personalised care ensuring that patient centred programmes of care are evaluated and managed within their sphere of competence and knowledge.

Main duties of the job

  • Ongoing responsibility for the management of a defined caseload
  • Accept referrals from health and social care professionals and voluntary organisations and self referrals within a defined population
  • Responsibility for the organisation and management and development of the Community care team
  • Responsibility for the assessment, implementation evaluation and reassessment of care to all patients referred to the team
  • Plans programmes of care for patients with diverse and complex health needs
  • Primarily visits patients in their own homes or another place of residence eg: residential care home
  • Responsible and accountable for delivery and organisation of clinics to meet the changing needs of the service, eg: dressing and phlebotomy clinics.

Clinical knowledge, skills and leadership

Essential criteria
  • Registered Nurse qualification
  • Specialist Community Practitioner – District Nurse Qualification.
  • Evidence of post graduate masters level study or equivalent.
  • Independent Nurse Prescribing Qualification V300 (commitment to completing within 2 years)
  • Extensive experience of working autonomously within a community setting
Desirable criteria
  • Advanced clinical assessment skills
  • Leadership and management qualification
  • Safeguarding qualification
