Specialist Physiotherapist - Community Stroke Team

Job overview

Are you passionate about working in stroke rehabilitation? Would you like to work in a community stroke rehabilitation team, working with patients who have recently been discharged from hospital following an acute stroke.  If the answer to the above is yes, then we would like to hear from you!

The hours are 22.5hrs (3 days) permanently with an additional 7.5hrs (1day) for 12 months. The post is based at Knaresborough Road Health Centre and is part of the wider Stroke neurology and Amputee therapy service. 

Due retirement we have this physiotherapy post for someone to work as part of the small MDT service including another physiotherapist, occupational therapists and rehabilitation Practitioners. We support patients to achieve their optimal functional level and to live successfully in the community.

Main duties of the job

·           Full training for staff will be given, to support becoming specialist in the relevant skills.  Staff must have a min of 2 years clinical experience to build on/develop including working in the NHS.

·           To carry an agreed clinical caseload as part of the CSNT. To provide specialist physiotherapeutic assessment, diagnosis, treatment and advice to patients who may have diverse and complex presentations (stroke and neuro rehabilitation).

·           To supervise and train junior staff in stroke rehabilitation.

·           To work independently under the guidance of the advanced physiotherapist ensuring active participation in the professional development of self and others.

·           To contribute to other departmental activities.

·           The CSNT case load requires visiting patients in their place of residence and provide specialist input. This may include visiting the Stroke unit (Granby ward) to review patients who are being imminently discharged.

·           Contribute to the SSAP date audit set

·           To liaise with and support in-patient Stroke Team – Granby ward

·           With support, take a major role in the relevant service developments for the CSNT service.  Be an integral member of the service development for CSNT and stroke service

·           To participate in a rota to cover the acute stroke unit over bank holiday weekends. In future should there be a change in the service, to work across 7 days as part of CSNT.

Criteria

Essential criteria
  • BSc (Hons) Physiotherapy or equivalent
  • Completed a range of post-grad training on relevant subjects
  • Minimum 2 years post graduate experience
  • Experience of rehabilitation in-patient and/or out-patients
  • Skilled in a wide range of physiotherapy assessments in a variety of settings
  • NHS experience min 18 months
Desirable criteria
  • Evidence of service development
  • Completed or working towards professional practice educators qualification
