We are excited to invite applications for Specialist Familial Hypercholesterolaemia (FH) Nurses to join the Yorkshire & Humber Familial Hypercholesterolaemia Service — a dynamic, innovative, and region-wide nurse‑led service dedicated to improving the diagnosis and management of adults and children with Familial Hypercholesterolaemia and associated lipid disorders.

About the Service

The Yorkshire & Humber FH Service provides a comprehensive tertiary-level, nurse‑led provision across the region. The service is hosted by York & Scarborough NHS Foundation Trust, acting as the Lead Provider for a collaborative network that includes:

  • Calderdale & Huddersfield NHS Foundation Trust
  • Leeds Teaching Hospitals NHS Trust
  • Hull University Teaching Hospitals NHS Trust

Working closely with clinical leads at each of the four provider sites, our FH nurses deliver expert screening, treatment, and specialist advice to support patients and families living with FH across the region.

The post holder will be employed by York and Scarborough Teaching Hospital with Hull as their base.

Main duties of the job

The post holder will provide specialist nursing advice and support to patients referred into the Familial Hypercholesterolaemia (FH) pathway within their local provider site. They will deliver a nurse‑led service to identify index cases and coordinate family screening for both adults and children. Working collaboratively with local and regional multidisciplinary teams, the post holder will contribute to all aspects of the FH service, including pathway delivery, service set‑up, monitoring outcomes, quality improvement and producing reports for stakeholders.

They will demonstrate autonomy, professional leadership and initiative to drive ongoing service development. Using advanced clinical, educational and management skills, the post holder will ensure a seamless, patient‑centred service across organisational and professional boundaries. They will actively engage with MDTs, primary care and national partners to support evidence‑based practice, education, research and audit, ensuring high‑quality, coordinated care for patients and families.
