Job overview

We are looking for enthusiastic and passionate District Nurses with excellent communication skills and great leadership skills to motivate and support others to maintain the delivery of high quality, proactive and safe care that we at HDFT strive to achieve.

Main duties of the job

To lead and manage a team in the provision of an community nursing care for a defined locality

Establish high quality programmes of evidenced based care for individual patients referred to the team.

Ensure the implementation and evaluation of care in conjunction with other agencies incorporating statutory and voluntary services

Line manage a team with varying skill mix

  • Registered Nurse with NMC registration
  • Post Reg District Nursing Qualification
Desirable criteria
  • Degree in Community Health Care Nursing

Experience

  • Post qualification community experience
  • Experience of managing caseloads
Desirable criteria
  • Experience of working with patients with long term conditions

Skills

Essential criteria
  • Excellent communication skills (written and verbal)
  • Clinical Supervision/Leadership/Supervisory Experience
