This is an exciting opportunity for an enthusiastic, motivated Band 5 Community Nurse  to join a dynamic and developing patient focused rehabilitation service, based in York and supporting people in their own homes.

Prior understanding of Community nursing is desirable . Further competency training and development is expected and will be supported. You will be required to work closely with the Out of Hospitals Directorate team and will have undertake a wide variety of tasks.

You will have excellent communication and organisational skills, work flexibly and be a strong team player. You be providing support to students and junior / non registered members of staff.

Main duties of the job

Our service provides planned and time limited treatment, rehabilitation and care to patients in their own homes, maximizing independence, preventing unnecessary hospital admissions, and facilitating hospital discharges.

Working as part of an established team, you will be helping individuals stay independent by receiving rehabilitation and care in their own home, rather than hospital or care facilities. Significant training and development opportunities will be available.

You will be responsible for providing holistic assessments as well as specialist nursing treatment and management of patients, providing rehabilitation to patients as part of a Multi-Disciplinary Team, within the Selby locality.

We provide a seven day service between the hours of 8am - 8pm. The successful candidate will take part in a rota that covers all hours of the service, however we do offer a flexible and varied shift pattern to staff.

We work closely with all partners, including: hospital wards, GP’s, A&E, Social Services and the voluntary sector, supporting hospital discharges and preventing unnecessary admissions.

The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy.
