Job overview

We are looking for an enthusiastic and passionate individual to join our team as the Liver Cancer Pathway Navigator.

The successful candidate will work alongside operational and clinical teams, to support the Hepatology Team to ensure regular surveillance of patients with Cirrhosis at risk of developing Liver Cancer  across the Trust's region, and will be instrumental in proactively managing and escalating patient pathways. The role also forms a key point of contact for patients on these pathways, by providing information and support to address patient needs, improve the patient experience and support patients to attend appointments where necessary.

You will be required to liaise regularly with colleagues at all levels of the Trust, and must possess the ability to work autonomously and respond quickly to requests, from both patients and clinical & operational colleagues. It is important that you can adopt a calm, caring, and sensitive approach when providing appropriate support and signposting to both service users and staff.

The post will be based at York Hospital and remotely be part of a team that works across the Humber and North Yorkshire area.

Main duties of the job

The post holder will act as a main point of contact for patients and carers, in coordinating and managing high quality patient care for patients on the Cirrhosis database.  

The candidate will be responsible for managing the database, ensuring that records are kept up to date and patients are invited for surveillance within the correct timeframes. The candidate will also play a part in developing better ways of working and improvements to the pathway and patient management.  

The pathway navigator will be expected to confidently manage information, using approved tools and procedures and have excellent communication and organisational skills. 

Working closely with the Hepatology Clinical Nurse Specialist (CNS) team, you will have a key role in ensuring that patients have surveillance on time, supporting those patients through their investigations and acting as a key point of contact. 

The post holder will be responsible for supporting the coordination of the patient pathway and the associated administrative functions; this may involve arranging investigations, appointments, and Multi-Disciplinary Team discussions in a timely manner to ensure the patient’s needs are met.
