Heart failure doesn’t mind your age or what you look like…..and its on the rise. Would you like to be part of a dynamic team exploring ways of supporting our patients living with heart failure?
There is now a great opportunity to join the team to help change and shape the future of heart failure nursing management across the Vale of York. We have been piloting new ways to improve management of heart failure patients, both within the acute setting and delivering innovative care within the community. Can you see yourself pivotal to change and development?
This post will allow us to expand and develop the services we offer. For you this will mean an opportunity to be involved at the front of an innovative service, progressing and developing knowledge and skills in heart failure whilst working alongside committed and compassionate colleagues.
Support will be given to work towards heart failure specialist nurse competency framework set by the British Society of Heart Failure
The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.
Main duties of the job
The post holder will be part of a team of experienced heart failure specialist nurses and be responsible for providing high quality expert care to patients with heart failure in York hospital, and the Vale of York community setting. The heart failure specialist nurse team deliver an out of hospital rapid access heart failure clinic based at a local health centre in central York, community clinics in a variety of community settings across the Vale of York and home visiting to housebound patients within their own homes. The team are also building a heart failure in-reach model, which focuses on admission avoidance and early step down to community delivered care for patients presenting too or currently being managed within York Hospital.

Essential criteria
- Excellent communication and interpersonal skills capable of communicating highly complex and highly sensitive or contentious information to patients and carers including where there may be barriers to understanding.
- Ability to organise own workload and prioritise effectively.
- Ability to work autonomously and evidence of flexibility, innovation and problem solving
Aptitude and Personal Qualities
Essential criteria
- Ability to identify and develop own and others knowledge and skills
- Car driver, full licence and access to own transport
- Excellent verbal/communication skills and ability to keep contemporary records
Education, Qualifications and Training
Essential criteria
- Professional qualification e.g. registered nurse
- Degree level specialist training or equivalent cardiac experience.
- Non medical prescribing qualification
Desirable criteria
- Masters degree level study
Experience and Knowledge Required
Essential criteria
- Experience of managing cardiac patients at a senior level
- Experience of development of procedures and protocols
- Experience of change management

- Highly specialised knowledge relating to Heart Failure
