We invite you to consider an exciting opportunity to join our multidisciplinary team on the Virtual Frailty Ward at York and Scarborough Teaching Hospital. 

The Virtual Ward aims to reduce hospital length of stay and unnecessary hospital admission, delivering acute care to patients in the comfort of their own home, providing assessment, diagnosis, acute intervention and care. You will work closely within a multi-disciplinary team alongside system partners, including the Urgent Crisis Response and front door teams. 

The Virtual Frailty Ward is a cutting-edge service designed to provide high-quality, patient-centred care to individuals living with frailty. Our goal is to help patients maintain their independence and improve overall quality of life, reducing hospital admissions or expediting their hospital discharge.  Your clinical expertise, compassion, and adaptability will be essential in supporting patients and their families throughout their care journey.

Our Virtual Ward is expanding in line with national guidelines, operating an 8am – 8pm service, 7 days per week.

Main duties of the job

• To act as a role model for the development of clinical skills and knowledge within the virtual ward team.

• Support the Ward Leader in setting and monitoring standards, and seek to continuously improve standards and the quality of patient care.

• In conjunction with the Ward Leader, ensure management of clinical services and efficient deployment of staff according to service needs.

• Support the Ward Leader in staff rostering, ensuring adequate skill mix.

• Support the Ward Leader in investigating and responding to accidents, complaints, untoward incidents and other significant events.

• Deputise for the Ward Leader in their absence.

  • To provide assessment, planning, implementation and evaluation of nursing needs for patients under the care of the Virtual Ward

·       Interviews for this position will be held in person ONLY at (hospital site). Remote interviews will not be available.

·       The validity of Nursing and Midwifery Council PIN will be checked before shortlisting (Only applicable to registered roles) - inaccuracies in NMC PIN details will mean your application will not be progressed
