NHFT is looking for an experienced clinical pharmacist to lead and support the delivery of a clinical pharmacy service to their community inpatient beds.
The post holder will act as clinical lead for pharmacy services to Community Beds within NHFT.
Our community hospitals offer inpatient specialist services for adult patients who need 24-hour inpatient care. The service is for sub-acute medicine and a combination of medical, physical and neurological needs.
The patients may:
• Have had a severe illness
• Had a decline in their health after a long-term illness
• Need medical, physical or specialist rehabilitation
• Have had a stroke or a brain injury
• Require end-of-life care
This is a varied role with the successful applicant needing to work independently and within the pharmacy team. Travel between sites will be necessary and you must be able to travel independently to other bases in the Trust across Northamptonshire.
Main duties of the job
As the Advanced Pharmacist Elderly Care, Rehabilitation and Frailty your duties will include:
• To be responsible for leading, delivering and evaluating clinical pharmacy services to patients within Community Beds
• To provide a clinical pharmacy service to Community Beds inpatient areas, working with other pharmacists and pharmacy technicians within the team
• To provide expert advice on pharmaceutical matters, undertaking risk management and ensuring compliance with medicines legislation and local policy
• To facilitate and support the efficient and cost-effective use of medicines
• To be part of the Pharmacy team and work with staff within the Pharmacy department and across the Trust, to enable the objectives of the Trust to be achieved
• To cascade to the members of Community Beds decisions made by Medicines Management Committee
• To contribute to the Trust compliance with Controlled Drugs, Pharmacy procedures, Medicines Management standards, Clinical Governance and Risk Management

Qualifications
Essential criteria
- Registered with GPhC as a Pharmacist
- MPharm degree, or equivalent pharmacy qualification
- Post-graduate diploma in clinical pharmacy (or equivalent level of knowledge and experience)
- Evidence of recent continuing professional development
Desirable criteria
- Member of Royal Pharmaceutical Society
- Independent prescriber
Experience
Essential criteria
- Post registration experience within the NHS setting
- Experience of working as part of a multidisciplinary team
- Experience of planning, implementing, analysing, reporting and changing practice through audit
- Experience of teaching, mentoring and tutoring
- Experience of planning, implementing, analysing, reporting and changing practice through audit
- Experience of data collection, analysis and report writing
Desirable criteria
- Experience of prescribing within a specialty to optimise medicines for specific patient groups
- Management qualification relevant to the NHS
- Experience of developing and implementing medicines management policies and procedures
- Experience of managing staff
