Are you ready for a role that makes a real difference?

At NHFT, we’re looking for a Clinical Pharmacist to lead and support the delivery of a clinical pharmacy service to our community inpatient beds.

Our community hospitals offer inpatient specialist services for adult patients who need 24-hour inpatient care. The service is for sub-acute medicine and a combination of medical, physical and neurological needs.

The patients may:
•    Have had a severe illness
•    Had a decline in their health after a long-term illness
•    Need medical, physical or specialist rehabilitation
•    Have had a stroke or a brain injury
•    Require end-of-life care

Admissions typically follow an acute hospital stay, helping patients regain independence and avoid unnecessary hospital readmissions.

 

This is a varied and autonomous role where you will work both independently and as part of the wider pharmacy team. You’ll have the opportunity to shape clinical pharmacy services, implement pharmacist prescribing, and make a real difference to patient outcomes.

·       Travel between sites is required, so you must be able to travel independently across Northamptonshire.

Main duties of the job

As the Advanced Pharmacist for Elderly Care, Rehabilitation and Frailty you will:    

  • Lead and deliver on-site clinical pharmacy services across our community bed inpatient wards
  • Lead, evaluate, and improve clinical pharmacy services for patients in these units.
  • Conduct medicines reviews to rationalise complex regimes and optimise therapy.
  • Provide expert advice on safe and effective use of medicines within the inpatient areas.
  • Prescribe and adjust medication (where qualified), within agreed competence, and monitor patient progress.

  • Strong clinical knowledge and ability to work autonomously.
  • Excellent communication skills and a collaborative approach to working within multi-disciplinary teams.
  • A prescribing qualification (desirable but not essential).

Qualifications

Essential criteria
  • Registered with GPhC as a Pharmacist
  • MPharm degree, or equivalent pharmacy qualification
  • Post-graduate diploma in clinical pharmacy (or equivalent level of knowledge and experience)
  • Evidence of recent continuing professional development
Desirable criteria
  • Member of Royal Pharmaceutical Society
  • Independent prescriber

Experience

Essential criteria
  • Post registration experience within the NHS setting
  • Experience of working as part of a multidisciplinary team
  • Experience of planning, implementing, analysing, reporting and changing practice through audit
  • Experience of teaching, mentoring and tutoring
  • Experience of data collection, analysis and report writing
Desirable criteria
  • Experience of prescribing within a specialty to optimise medicines for specific patient groups
  • Management qualification relevant to the NHS
  • Experience of developing and implementing medicines management policies and procedures
  • Experience of managing staff
