Job overview

This is an exciting opportunity to join the enthusiastic team of the Department of Digestive Diseases which is evolving to provide a modern responsive service for the needs of patients with Gastroenterological disorders in Northamptonshire and South Leicestershire. We have a Consultant of the Week model of care for in-patient services.  Our endoscopy department has been refurbished and expanded to a well-equipped five room unit.  

New appointee will join our seven existing consultants in delivering a wide range of inpatient and outpatient services. The successful candidate will work with the current junior staff.  This consists of 3 StRs, 1 Trust grade registrar, 2 x IMT, 1 x GPVTS, 2 x F2s, and 1 x F1. There is also a doctor’s assistant to help the ward-based doctors with administrative tasks.

We are seeking motivated and enthusiastic colleagues with a broad range of GI skills with primary interest in Luminal /Nutrition, to join the team to push the boundaries further and improve the timely delivery of a high-quality service.

We all appreciate that consultants develop their skills with practice and the Trust have a proactive approach in encouraging and supporting the development of the leadership and management skills of its consultants.

Main duties of the job

  • Maintenance of the highest clinical standards in the management of Gastrointestinal patients and uphold the principles of GIRFT.

 

  • To share with colleagues the responsibility for the day-to-day management of the patients on Digestive Disease Unit ward (Consultant of the week model)

 

  • Provide both diagnostic and therapeutic upper GI endoscopy and colonoscopy.

 

  • Focus on the delivery of core Gastroenterology services.

 

  • Subspeciality interest in Nutrition is desirable to support the existing team. Subspeciality interest in Hepatobilliary (ERCP / EUS) is also desirable to support the existing team and develop an EUS service.

 

  • Participate in “GI bleed” oncall rota.

 

  • Teaching and training of junior medical staff, nurses, and medical students from the University of Leicester.

 

  • To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.

 

  • To have responsibility for ensuring active participation in continuing medical education (CME), mandatory training and appraisal/revalidation.

 

  • Engage with and encourage research participation relevant to the special interest area of the post holder.

Detailed job description and main responsibilities

The Digestive Disease Unit ward is the base ward for the Gastroenterology and Hepatology team and has 20 beds and a 3 bedded day-case unit. There is a consultant of the week rota (CotW) and the CotW is responsible for overall care of the patients on DDU (mostly GI/Liver/Nutrition related patients with some general medical patients) and provide supervision for registrars who cover referrals from other departments; provide the appropriate clinical and educational oversight of the ward junior doctors and work with the multi-professional ward team to deliver safe and high-quality care of the patients. The trust’s values and GMC’s GMP principles are the guiding principles. The CotW also is the first port of call on the Consultant Connect telephone advice service for local GPs.

 

Endoscopy service provision would be expected to include diagnostic and therapeutic procedures, both in Upper and lower GI endoscopy. Advanced endoscopic skills would be encouraged and there is provision to support the capsule Endoscopy services for those with the skills. Other specialist endoscopic skills like ERCP/EUS are not mandatory but appropriately trained post-holders would be welcome to provide the services.

 

The post has a luminal gastroenterology and nutrition focus and would join the current teams 1 academic, 3 fulltime and 2 part time consultants and 4 specialist nurses to provide a high quality luminal and nutrition service. Responsibility for the proper functioning of the department and any responsibility that relates to a special interest would need balancing with clinical need. This includes ongoing management of inpatient and outpatient care of general Gastroenterology and General Medicine patients.

The appointee would be expected to deliver out-patient services including “2-week wait” Gastroenterology and Hepatology referrals and take ongoing responsibility for patients under his/her care. He/she would also provide a consultation service and advisory service to clinical colleagues in other specialties within the Trust and Primary Care.

 

The consultant appointee would form part of the “GI Bleed rota”.

Qualifications

Essential criteria
  • Entry on GMC Specialist Register; eligible for entry on Register or within six months of receipt of Certificate of Completion of Specialist Training (CST) at time of interview
  • MRCP or equivalent
Desirable criteria
  • Subspeciality accreditation in Nutrition
  • Postgraduate qualification

Clinical Experience

Essential criteria
  • Clinical training and experience equivalent to that required for gaining UK CST in Gastroenterology and General Internal Medicine.
  • Ability to offer expert clinical opinion on a range of problems within Gastroenterology and general medicine
  • Ability to take full and independent responsibility for clinical care of patients
  • Provide evidence based clinical care
  • Demonstrate JAG competencies for upper GI endoscopy and colonoscopy
Desirable criteria
  • Five years supervised training in an appropriate StR equivalent training programme
  • Demonstrate a high level of endoscopic performance data
  • Ability to perform high level endoscopic therapeutics ie. EMR
  • Advanced training in Nutrition
  • Experience in managing patients with complex nutritional needs
  • Previous experience of participation in research projects

Management and Administrative Experience

Essential criteria
  • Ability to contribute to the efficient & smooth running of Gastroenterology departments.
  • Ability to organise and manage out and inpatient priorities, investigations and treatment.
  • Experience of audit management
  • Ability and willingness to work within the Trust and NHS performance framework and targets.
  • Work within a performance managed environment.
Desirable criteria
  • Previous experience in management/organisational role
  • Ability to manage and lead a sub-speciality in Gastroenterology.

Teaching Experience

Essential criteria
  • Experience of supervising F1, F2, CTs, Specialty Registrars (STs)
  • Ability to teach clinical skills
  • Undertake portfolio appraisal of junior medical staff.
Desirable criteria
  • Higher degree in medical education
  • Train the trainer’s qualification

Research Experience

Essential criteria
  • Ability to apply research outcomes to clinical problems
Desirable criteria
  • Publications in peer-reviewed journals
