Each day we deliver the health and social care services that people in Northamptonshire receive, in the community, at home, work or in schools.
In October 2022 we launched a new way of working across the site in Turn Furlong (Kingsthorpe, Northampton) , to provide a new integrated health and social care model of care for people needing short term support upon discharge from hospital.
The ‘Recovering Independence Bed’ units, is delivered by a range of staff from health and social care settings, to deliver clinical care, rehabilitation support, and care support, ensuring patients experience the best quality of service they can, in a prompt and smooth fashion.
The service provides a unique opportunity to work with a dedicated team, with experience from both elements of health and social care, to deliver vital intermediate care for patients in Northamptonshire.
You will need to effectively manage the safe and timely admission and flow of patients into the centre.
You will be working as part of an integrated multi-professional team that includes nurses, therapists, medical staff, senior managers and social care colleagues.
You will work closely with external health organisations, such as secondary care, social care and other internal NHFT care providers to ensure the safe and timely admissions to an appropriate bed that can meet the needs of the patients.

As a Patient Admissions Co-ordinator you'll
- Actively manage the safe and timely admission and flow of patients into the Recovering Independence Beds.
- Work closely with both internal and external healthcare providers to ensure a smooth transition of patients between different healthcare settings.
- Carry out pre-admission screening which involves interpreting blood and radiography results to ensure patient safety is maintained ensuring the receiving centre can met the needs of the patients
- Assist senior colleagues by providing support to junior colleagues and assisting them in problem solving relating to patients.
- Assist in developing new ways of working in the centre and developing strong links with other services and the wider community
- Need to be well organised, confident and able to be adaptable and resourceful to deliver high quality patient care.
- Be a key part of the team and work without any direct supervision on a daily basis with the ability to prioritise work at all times based on the patient need.
- Must have good IT skills using a variety of different platforms
You must be registered with the NMC and have some post graduate clinical experience.
