We seek dynamic nurses or advanced paramedics with amazing interpersonal skills to support the leads in the county wide Urgent Community Response team. We are looking to recruit new team members who can proactively and effectively deliver a range of treatments, including IV therapy and all aspects of clinical care in the patient’s own home environment.

The Urgent Community Response team operates as a Virtual Ward across the county, managing patients with sub-acute conditions to aid their recovery from illness or injury within their own homes by preventing unnecessary hospital admissions and reducing length of hospital stay.

In order to respond to the healthcare needs of the local population, the teams operate 7 days a week ensuring the delivery of the service can respond to patients’ needs as and when required. The shifts for the service are between 07.00am to 01.00am including weekend and bank holiday working as part of the rota. The successful candidates will be expected to work within these shift patterns. 

Main duties of the job

We are looking for experienced RN’s or advanced paramedics with a passion for high quality clinical care and also experience in either acute medicine, Emergency Care or Community Nursing. The post holder will provide acute assessment & interventions in patients’ own homes and will ensure that patients are referred onwards as appropriate to community services once their care episode is complete. The vision of the service is the key to bridging the gap between hospital and community - bringing care closer to home and we also provide dedicated nurses into the front doors of both our acute hospitals in the county.

To be considered You must  have:-

• Excellent organisational ability
• Senior nurses/paramedics band 6/7  with excellent leadership skills
• Superb level of communication skills
• Able to flex to a changing environment for service improvement
• Good working knowledge of digital communication products
• Real interest in improving & delivering healthcare in the community.
• Clinical triage skills
• High level assessment, diagnostic and clinical skills
• Prescribing or willing to work towards
• Be capable of working under pressure across a variety of  treatments
