Job overview

Meadowbank is unique in that it supports development of skills for community survival in an environment that aims to replicate  the demands of every-day living, including the opportunity to explore employment.

People who may benefit from rehabilitation will be identified as early as possible within their care pathway to enable the development of positive relationships and a thorough understanding of that person’s needs.

 

In order to support a seamless transition for the individual the post holder will be working in an integrated skill mix team, including Psychologist, Occupational Therapist and other qualified clinicians  and will help facilitate close liaison with other community mental health and  in-patient services the Local Authority, and a wide range of non-statutory service providers.

 

This  role requires flexibility, personal resilience, working under pressure and an ability to empathise with and support people who have complex needs.  

Main duties of the job

The primary focus  to enable a recovery focussed individualised approach for rehabilitation through to community services. The role will be to support the clinical team to coordinate and define the  rehabilitation needs  of individuals alongside the Multidisciplinary team. Support needs and individual interventions to optimise recovery will  be defined by drawing on innovative and evidence based models of practice and assessment tools, such as the Life Skills Profile and  CHIME.

Detailed job description and main responsibilities

·       Lead, coordinate and support the team to carry out clinical duties ensuring delivery of high quality rehabilitation care

·       Support the Unit Manager and Service manager in the management and administration of the Unit.

·       Support and encourage professional and personal development of recovery and rehabilitation competencies 

·       Support the team to review goals for recovery and rehabilitation using identified tools/models  such as the Life Skills Profile and CHIME

·       Support the team to work with service users  to achieve their prioritised recovery goals and improve their quality of life based on clinical and patient reported outcome measure  e.g. ReQol’s

·       Be a key participant in the Multidisciplinary assessment and admission of individuals to the unit

·       Participate in the nominated Clinical Team Lead  rota for the Berrywood site.

Person specification

Knowledge and Experience

Essential criteria
  • • RMN or RN Dip/HE (Mental Health). • Experience post registration for 18 months minimum. • Experience of Adult Acute inpatient nursing ( in past 18 months). • Post qualification/registration experience in adult mental health.
Desirable criteria
  • • Previous experience in the Supervision and Assessment of students • Experience of rehabilitation and recovery • Caseload management experience • Understanding of wide rang of mental health conditions

Skills and abilities

Essential criteria
  • • Able to work collaboratively within a team structure. • Flexible with regard to shift work. • High clinical standards. • Leadership/management skills
Desirable criteria
  • • Able to teach/supervise staff. • IT skills
