Primary Care Liaison Worker

Band 6

We are currently looking for motivated and enthusiastic Band 6 Nurses to join the multi professional staff team at the Urgent Care & Assessment Team (UCAT) based at Campbell House, Northampton.


The post holder will be required to provide mental health support across a group of GP practices. Working alongside GPs and other surgery staff the aim is to support the assessment, signposting and provision of information for primary care mental health assessments and queries which do not meet threshold for secondary care. To facilitate effective communication between secondary care and primary care services, to ensure Service Users receive the appropriate assessment and interventions in a timely manner.


You will have excellent assessment, risk management and communication skills. You will be committed to a recovery based approach promoting social inclusion. You will be enthusiastic about making links with primary care colleagues as well as other agencies.


Successful candidates will need to have genuine interest in working within a multi professional team providing assessments, short term interventions and treatment for the defined group of individuals and will have experience of working with people who have mental health difficulties.
