Job overview
An exciting development opportunity has arisen for a Band 7 Community Nurse in a new service that bridges Primary Care and Community Nursing in East Northants PCN .
This initiative aims to proactively identify and manage health needs, reduce health inequalities, and deliver tailored interventions directly in patients’ homes. By focusing on early identification, prevention, and long-term condition management, the service will enhance patient outcomes, reduce practice appointments, reduce hospital admissions, and support the broader goals of integrated community care and neighborhood working.
The successful candidate will need to demonstrate significant experience in the field of Community Nursing or Primary Care, together with a genuine desire to develop both the service and yourself.
They will use their management experience to lead on service changes and developments and contribute at a local level to the development and implementation of policies and procedures.
They will lead a small team, with involvement in service coordination, staff wellbeing, supervision, governance and service performance and improvement.
Main duties of the job

By focusing on early identification, prevention, and long-term condition management, the service will enhance patient outcomes, reduce practice appointments, reduce hospital admissions, and support the broader goals of integrated community care and neighborhood working.
You will be expected to see patients within a given geographic area and may mean that you will need to liaise with a variety of GP surgeries.
As both an organisation and a service we are actively involved both local and national innovative projects around improving quality of care, integrated working with stakeholders, designing new pathways. We have an extensive program of training and skills development available for staff and actively nominate and encourage our staff to apply for national awards and recognition, such as Queen’s Nursing Institute, RCN and Nursing Times awards.

Qualification
Essential criteria
- NMC Registration -Registered 1st level nurse
- At least one post registration relevant accredited clinical specialist course at degree level completed in the last 2 years e.g. CHD, Diabetes, Respiratory conditions, minor illness and minor injury.
- Nurse prescriber or working towards this.
Desirable criteria
- Accredited qualification in leadership or managment.
- Complex Case Management (LTC) qualification or experience in actively managing a caseload of patients with LTCs
- Relevant postgraduate study at masters level
Experience / Knowledge
Essential criteria
- Demonstrable experience in leading and management a team.
- 2 year post qualification experience in Community Nursing or Primary Care at at least band 6 level
Desirable criteria
- Required to record patient related information both electronically and in the written form, both in an office base and the patient’s home.
- Working with community development initiatives
- Knowledge of change management tools and able to demonstrate ability to influence change and affecting change through others.
- Demonstrated in the application form knowledge of HR policy and process
- Evidence of inputting into the patient held record.
Skills
Essential criteria
- Indicates an understanding of individual values, and how they impact day to day working practices.
- Shows aptitude for the use of range of IT software (capable of producing reports/documents/presentations as appropriate).
Desirable criteria
- Able to identify determinants on health in the area, knowledge of public health issues
- Able to work in fast paced environments.
- Demonstrates aptitude to work autonomously as well as part of a team.
Communication
Essential criteria
- Excellent interpersonal and intrapersonal communication skills
- Approachable and accessible.
Desirable criteria
- Able to challenge appropriately.
- Has an appreciation of different styles of communication.
