The post holder would provide quality rehabilitation for a group of patients, those stepping down from the acute hospitals or stepping up from the community to avoid an acute admission.

Our ward specialises in a mixture of stroke rehabilitation and general complex rehabilitation. The therapy team work in the Howard Johnson therapy Hub- which consists of a large gym, assessment kitchen and other working spaces, for computer access for our electronic notes and onward referrals.  We work as part of the wider Hazelwood ward team, working on the ward, the therapy garden and the Howard Johnson therapy Hub.

Working for our organisation

NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible.
