Job overview

We are looking for motivated and compassionate Band 3 Healthcare Assistants to join our Hospital at Home / Virtual Ward team in Mid Nottinghamshire. In this community‑based role, you will support people with acute health needs to remain safely in their own homes by providing person‑centred care, rehabilitation support and medical monitoring. Working as part of a multidisciplinary team, you will help deliver hospital‑level care in the community and contribute to preventing unnecessary admissions.

This role is ideal for someone who is adaptable, reliable and confident working independently in patients’ homes. You will receive full training in medical monitoring and delegated clinical tasks, with opportunities for ongoing skills development and career progression. The role involves regular travel across Mid Notts and wider when required, clear and sensitive communication with patients and families, and the ability to respond flexibly within a fast‑paced, patient‑focused service.

Main duties of the job

Under the direction of a registered professional, contribute to high‑quality care by supporting assessment, planning, implementation and evaluation.
Provide personal care, rehabilitation and reablement in line with individual treatment and care plans, helping people to improve their function, independence and confidence.
Undertake delegated medical monitoring and collect accurate clinical and functional information to support safe decision‑making and demonstrate the impact of interventions.Work at times without direct supervision in patients’ own homes, residential homes or nursing homes, ensuring safe, compassionate and dependable practice.
Work collaboratively with the Urgent Community Response (UCR) team to prevent avoidable acute admissions by supporting people to remain safely at home.
Support the Discharge to Assess pathway by contributing to timely, safe hospital discharge and helping patients maintain or improve their level of function and quality of life.
Work within a Virtual Ward (Hospital at Home) model, contributing to remote and face‑to‑face monitoring, supporting clinical decision‑making and enabling hospital‑level care in the community.

Detailed job description and main responsibilities

·         The post holder will assist patients with their personal, physical and emotional needs, within the overall programme of care as directed by the registered professional. 

·         To undertake nursing, therapy and social support activities following training and demonstration of competency, delegated by the relevant profession.

·         To work without direct supervision undertaking tasks delegated by the relevant profession in order to support the team.

·         To deliver high quality care to patients within the home and clinic setting, respecting patients.

·         To ensure that consent is obtained before any intervention is implemented.

·         Respect the patient and carers as individuals always respecting privacy and dignity.

·         Participate in administering medication, hard collar care, stoma care and eye drops.  This list is not exhaustive, but all training and competencies will be offered.

·         Provides advice which contributes to the care, wellbeing or education of patients, including Health Promotion.

·         Maintain confidentiality of all information gained during course of duty.

·         Work flexibly to meet individual health care needs of the patient and carer.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. 

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

  • NVQ Level 3 qualification (or equivalent) in a relevant health & social care area

Experience

  • Experience of working in a healthcare setting.
Desirable criteria
  • Experience working in an adult rehabilitation / caring role
  • Previous wound care / clinical skills

Knowledge

  • To have a basic understanding of the different types of physical illness and maintain an understanding and caring approach to patients’ needs.

Skills

Essential criteria
  • Excellent verbal / written communication and interpersonal skills
  • Developed observational skills with the ability to report and record physical / mental health observations of incidents / events and behaviours in a timely and accurate manner.
  • Good time management skills
  • Able to deliver the principal accountabilities identified in the job description in a variety of settings.
  • Can demonstrate the ability to communicate and support individuals with life limiting conditions.
  • Ability to communicate with patients and carers who may have cognitive impairment.
  • To be able to deal with challenging family situations or circumstances, including care of the dying.
Desirable criteria
  • Knowledge and experience of a range of clinical skills, including venepuncture, ECG, Catheter care, Wound care.
