Following national and local guidelines, plan, organise, deliver, and evaluate comprehensive health assessments to assess and formulate the complex needs of a looked after child or young person using evidence-based knowledge to develop health plans toward improving the long-term health outcomes for looked after children. 

To provide clinical advice on the provision of care across the specialism of Children in Care and Adoption.

To provide realistic, timely support, as appropriate, to children and young people in the care of the local authority.

To work in partnership with the Named Nurse for Children in Care, Clinical Nurse Specialist for Children in Care and Designated Professionals for Children in Care.

To empower children and young people to take responsibility for their own health and wellbeing 

To interpret and apply relevant, up to date research to clinical practice.

The Children in Care health team provides holistic, child/young person-centred health assessments to improve health outcomes for children who are in care of the local authority. 

The post holder will ensure collaborative working across Nottinghamshire embedding best practice for children in care with effective use of resources and integrated working with the 0-19 service. 

The individual will need to be highly motivated, with excellent communication skills and with the ability to ensure the delivery of high-quality services and drive forward service improvement within a partnership culture.

To coordinate health assessments for Children in Care in collaboration and partnership with Children’s Services/Children and Young people’s Services.

To act as a resource for Social Workers, Foster Carers and Young People.

To work flexibly with children in care, in conjunction with the Clinical Nurse Specialist arranging their health input according to their health needs and choices.

To develop and maintain links with specialist health services e.g. Child and Adolescent Mental Health, Substance Misuse Services, Sexual Health and Acute Services. 

To provide General Practitioners, Children’s Services/Children and Young People’s Services and other health professionals with relevant information bearing in mind issues of confidentiality. 

To participate in interviews and discussions regarding children in care as appropriate. 

Work in collaboration with the Clinical Nurse Specialist for Children in Care, Designated Doctor and Paediatricians to maintain an overview of the child and young person’s health needs during their journey through care. 

To act as resource for children and young people and their carers in developing their understanding of the responsibilities for their present and future health needs.

To support the Clinical Nurse Specialist for Children in Care and Designated Doctor to develop evidence based practice and implement this with regards to the health needs of children in care. 

To work in close partnership with Primary Care and Children’s Services to promote the health and wellbeing in the best interest of children in care.

To participate in the training of Children Services and the Primary Health Care Team in conjunction with CAMHS, and Children’s Services as part of the Training Strategy Programme on understanding and managing the health needs of Children in Care.

To actively support the accurate collection of data through appropriate use of SystmOne in order to be able to report on the key performance indicators for the service.

Act in accordance with the Data Protection Act and Information Governance Policy to ensure confidentiality and protection of client information. 

To maintain accurate, up to date, contemporaneous records in line with NMC record keeping guidance and organisational policy. This includes SystmOne data entry on all activity and preparing reports for safeguarding and court purposes. 

To access and participate in Safeguarding supervision as per organisational policy. 

To access and participate in clinical supervision as per organisational policy

To actively engage patient and public representation in service monitoring and development.

To undertake mentorship, preceptorship and clinical supervision as appropriate to contribute to staff development and monitoring performance.

To work within NMC, National Organisational Policies including Health and Safety as appropriate.

To be pro-actively involved in service developments, training and innovation.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. 

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

  • Significant experience of working with children and young people in disadvantaged circumstances (trauma, poverty/ deprivation / abuse / neglect / Domestic Violence)
  • Experience of assessing children and young people's physical and emotional health needs and developing healthcare plans to address these needs
  • Evidence of working in partnership with different agencies and across organisational boundaries
  • Experience of working with emotional health, sexual health, and substance misuse services
  • Working knowledge of safeguarding children issues and procedures, including wider contextual safeguarding issue – county lines, gangs, radicalisation, CSE, grooming, etc.
  • Experience of working in multi-disciplinary team
