The role of the NRC case manager is integral to the patient journey and interdisciplinary team. This role drives the coordination of admission, management of the patient journey and rehabilitation programmes, and discharge from the NRC whilst maintaining a key liaison role for families and carers. The post holder will be experienced in senior healthcare leadership, coordination of patient pathways, aligning professional and patient expectations and driving timely clinical decision making. The post holders will promote the clinically excellent model of rehabilitation at the NRC ensuring strong and visible clinical leadership throughout the patient journey.

Planning for the NRC is in its advanced stages, and the building will be ready in the summer of 2025. This role will be temporarily based at City Hospital until we move to the 70 bed NRC facility in summer 2025.

Qualifications
Essential criteria
- BSc Hons (or equivalent) Physiotherapy
- HCPC registration.
- Requires specialist knowledge in rehabilitation.
- Recognised post-graduate training relevant to specialty (e.g. either obtained MSc or completed modules
- Evidence of maintaining professional competence through CPD. Clinical Educators Course – relevant course to train/supervise students in own profession
Desirable criteria
- First line management course e.g. BELS
- Full membership of professional body
- Member of specialist interest group
Experience
Essential criteria
- Member of specialist interest group Experience of management of performance and sickness of staff and participation in recruitment of new staff to maximize workforce potential.
- Experience in evaluation of evidence-based practice, audit, service development and benchmarking
- Experience in use of appropriate outcome measures
- Awareness legislation relating to health and safety, litigation and Data protection.
Desirable criteria
- Band 7 experience in patients with complex rehab and high acuity needs.
- Experience of managing tracheostomy patients and acute respiratory patients.
- Experience of quality improvement and/or research.
- Experience in using technology within clinical practice delivery
- Awareness of Good Clinical Practice Guidance (GCP) for research

Communication And Relationship Skills
Essential criteria
- Excellent verbal, non-verbal and written communication skills including explaining, negotiating, motivating and training
- Participation and leading /organising in-service training programmes
- Present to groups of up to 25.
- Ability to build and maintain effective relationships with the multidisciplinary team, patients and carers
- Demonstrate leadership ability.
- Able to communication complex condition related information to patients, relatives/carers and other clinical/social care staff where there may be barriers to understanding e.g. language barriers, speech, cognitive problems
- To feedback constructive criticism with tact and diplomacy in performance management of staff.
- Able to maintain accurate and comprehensive treatment records in line with the trust policy and departmental procedures

Analytical & Judgement Skills
Essential criteria
- Ability to assess complex information, formulate a diagnosis and plan patient management using clinical reasoning skills and a range of treatment options.

- Ability to plan and contribute to the service delivery up to a year ahead and with the interdisciplinary team in own clinical area using available resources.
Physical Skills
Essential criteria
- Evidence of appropriate and relevant manual therapy skills to maximise rehabilitation and health outcomes
- Basic keyboard skills including the inputting of data.
- Ability to use a range of medical devices, some of which require dexterity and accuracy
Other Requirements
Essential criteria
- Undertake out of hours duties as required.
- Able to travel to required areas within and beyond the Trust
- Maintain competence in respiratory care where appropriate
Desirable criteria
- Openness and enthusiasm to work with new technologies to advance practice
