Job overview
The core purpose of this post is to take responsibility to provide high quality, patient-centred practice nursing care, within a GP practice.
The key focus and skills of the Practice Nurse will be to meet the nursing needs of the patients accessing appointments at Greendale Medical Centre; which includes long term condition management, screening, preventative care, patient education, wound care, clinical audits and the supervision of any less qualified clinical colleagues, while working in partnership with the Lead Nurse, GPs, management and administrative colleagues.
Main duties of the job
We are ideally seeking an experienced Practice Nurse. However, we also welcome applications from newly qualified or less experienced Nurses who are committed to developing their skills in primary care. A structured development and support package will be offered.

The Role
Practice Nurse
Practice Manager
£28,500 – 43,000 per anum (depending on experience)
June 2026
To provide safe, effective and patient-centered nursing care in general practice, supporting prevention,
patient education and the management of long-term conditions. The post holder will work as part of a
multidisciplinary team to deliver high-quality, equitable care.
Responsibilities
Clinical
Clinical Care
• Deliver a range of nursing care including wound care, dressings,
Contraception, venepuncture and immunisations
• Carry out health checks and screening, including opportunistic interventions making every contact
count (MECC)
• Assess patients and respond to clinical needs
Long-term Condition Management
• Support patients with diabetes, asthma, COPD and hypertension
• Deliver planned care in line with protocols
• Work with the multidisciplinary team to manage patient care
Patient Education and Prevention
• Provide clear advice on conditions and treatments
• Support self-management and independence
• Deliver lifestyle advice including diet, exercise and smoking cessation
Team Working
• Work collaboratively with the wider practice team
• Collaborate with neighborhood and community teams to support ongoing patient care and
improved health outcomes.
• Support development of Healthcare Assistants and junior staff
• Contribute to and promote a positive inclusive team culture
Governance and Safety
• Maintain accurate clinical records, using appropriate agreed codes, using agree standards of data
entry, templates, and protocols, completing all documentation, computerised and paper-based, as
is required for good patient care in a timely manner.
• Follow infection control procedures including handling of specimens, spillage of bodily fluids, sharps
injuries and disposal of waste materials in line with local policy.
• Ensure mandatory training is up to date including safeguarding and BLS
• Support audit and quality improvement activities
• Actively engage with ongoing QOF requirements, achieving quality indicators and targets to
support improved quality of patient care.
• Work in line with National policies and procedures, including NICE Guidelines, PGDs and PSDs.
• Ensure daily checks, fridge temperatures, drug monitoring and stock checks are completed and
uploaded to TeamNet.
• Work with Back Office to support call and recall processes.

Qualifications and Registration
Essential
• Registered Nurse (Adult) with current NMC registration
• Evidence of ongoing Continuing Professional Development
Desirable
• Primary care qualifications (e.g. diabetes, asthma, cytology)
• Teaching or mentorship qualification
Skills and Abilities
Essential
• Strong clinical skills including wound care, venepuncture and immunisations or willingness to
undertake training.
• Ability to assess patients and recognise deterioration
• Ability to support long-term conditions including diabetes, asthma and hypertension or
willingness to train.
• Strong patient education and communication skills
• Ability to promote self-management and lifestyle change
