We are a dynamic PCN based in Nottingham City, comprising of 3 member practices, with approximately 34,000 patients. We are committed to providing excellent care for our patients, as well as support for our staff.  

We are excited to be able to offer this opportunity for a PCN Care Coordinator (Safeguarding Support) to work closely with practice managers, safeguarding leads, clinicians, and external safeguarding agencies to ensure safe, effective, and consistent safeguarding processes are embedded within the PCN.

The post holder will also support PCN service delivery, project work, and personalised care in line with the NHS Long Term Plan and PCN Directed Enhanced Service requirements in our new, on-the-day, minor illness hub based at the same building as one of the member practices. Patients with minor illnesses needing an on-the-day, face-to-face assessment, will be booked in by the member practices according to agreed protocols.

Main duties of the job

The PCN Care Coordinator (Safeguarding Support) will provide coordination, administrative and safeguarding support to the Primary Care Network (PCN) . The role combines the functions of a Care Co-ordinator with dedicated responsibilities for safeguarding children and vulnerable adults.

The role requires excellent organisational skills, strong safeguarding awareness, and the ability to work both autonomously and as part of a multidisciplinary team. Must be able to work on the days that the three practices held there safeguarding MDT meetings .

Detailed job description and main responsibilities

Ensure safeguarding policies and procedures are up to date and aligned with national and local guidance.

Facilitate effective safeguarding multidisciplinary team (MDT) meetings, including intelligence gathering, documentation of decisions, and monitoring actions.

Support the preparation and submission of safeguarding reports for local authorities, child services, and other statutory bodies.

Assist practices in responding to statutory information requests (e.g., section 42 enquiries, case reviews).

Promote awareness of safeguarding referral pathways, including Early Help, MARAC, MAPPA, domestic abuse services, and victim support services.

Maintain and update a vulnerable patient register across the PCN. 

Support practices in achieving CQC safeguarding compliance.

Provide advice and support on recording and coding safeguarding information within patient records.

  • • NVQ Level 3 or equivalent qualification / experience in health or care.

  • • Knowledge of safeguarding processes for children and adults.
  • • Strong organisational skills, including planning, prioritisation and minute-taking
Desirable criteria
  • • Experience of working in primary care or within a PCN.
  • • Experience of coordinating or chairing MDT meeting
  • • Ability to audit and monitor compliance with safeguarding standards
  • • Knowledge of MARAC, MAPPA, MASH and Early Help referral pathways.
  • • Experience producing safeguarding or case review reports.
