Job overview

This is an exciting opportunity to join our dedicated Urology Department at King’s Mill Hospital. We are seeking a self-motivated, innovative individual with excellent attention to detail to support the delivery of high-quality patient care.

As an Apprentice Patient Pathway Support, you will play a key role in ensuring the smooth running of clinics by preparing and tracking clinic documentation, supporting the SFHT Patient Pathway and Patient Pathway Coordinators. You will help maintain a high standard of service across both King’s Mill and Newark Hospitals, contributing directly to the Trust’s goals.

Main duties of the job

  • Make appropriate preparations for clinics, to ensure that the patient's visit will run smoothly.
  • The prompt tracking in and out of case notes using the PAS system to maintain accuracy and completeness of all electronic records in accordance with Data Accreditation requirements.
  • Provision of typing support, as required, to support Pathway Coordinators.
  • Maintenance of good working relationships with the Patient Pathway Co-ordinators, liaising and raising issues as necessary and good relationships with patients and other members of staff.
  •  Ensuring adherence to the Trust’s confidentiality policy.
  • To deal with telephone enquiries from patients, relatives, clinicians and other members of Trust staff in relation to outpatient appointments and clinics.
  • To deal with cancellation of clinic appointments and rebooking of patients & bring patients appointments forward when required to support Patient Pathway Co-ordinator.
