Job overview
Sherwood Forest Hospitals NHS Foundation Trust is seeking a proactive and compassionate Discharge Support Officer to join our dynamic discharge team.
This role plays a vital part in ensuring the safe and timely discharge of patients, working closely with internal teams and external partners to support patient flow and continuity of care.
As part of the wider Corporate Capacity, Flow and Resilience departments, the discharge service is central to the Trust’s commitment to safe patient transitions. You will support the Discharge to Assess model of care and contribute to the management of:
- Patients with long lengths of stay
- Specialist discharge pathways (e.g. End of Life and Fast Track)
- Complex discharge plannin
You will also collaborate with colleagues across Adult Social Care, Nottinghamshire Health Care, and Community Services to ensure effective discharge processes.
Key Responsibilities:
- Support the discharge team in coordinating and facilitating patient discharges.
- Liaise with clinical teams, social care, and community partners to ensure discharge plans are safe and appropriate.
- Assist in identifying and resolving discharge barriers.
- Contribute to bed flow and virtual hospital functions where appropriate.
- Maintain accurate documentation and communication across multidisciplinary teams.
