The Home Assessment Reablement Team (HART) is a community based service that provides reablement support to service users from a wide range of hospital and community based settings within their own home across Oxfordshire. The service also supports service users in the community to prevent hospital admission. Close links have been developed with our community colleagues to ensure a smooth handover to any appropriate service and to prepare patients and clients for their future pathway.
The service facilitates the discharge of service users from acute and community hospitals who require some additional support or who are awaiting long term care packages/further rehabilitation at home.
Each service user has a goal based care plan which is designed to improve or maintain their functions by using an enabling approach to the delivery of care.
The service has three bases, Unipart House Business Centre (covers Oxford city), Horton General Hospital (covers North and West Oxfordshire) and Abingdon Health and Wellbeing Centre (covers South Oxfordshire). HART is looking to recruit to the North Oxfordshire and West Oxfordshire teams which are based at the Horton General Hospital
The Assessor role is key to ensuring that service users receive the appropriate and high quality care when they leave the hospital or have been identified as needing additional support after an episode of ill health.
The post holder will need to be organised, dynamic and bring a light hearted approach to a serious demanding role. You will be required to assess the service users in their own home or a hospital setting, initiate a goal based care plan, complete risk assessments along with health and safety assessments. You will also be required to review the support provided weekly in the service users own home. Knowledge of reablement and domiciliary care legislation would be an advantage.
You will manage a team of Reablement Support Workers within the community, therefore will need previous line management experience.
The role will be both office and community focused and full training and support will be provided.
The service operates 365 days a year from 7am–10pm. You will work on a rota basis including weekends and bank holidays.
It is essential that you hold a full UK driving license and have a car available for work at all times, due to the community based side of this role.
